(1) Background: The disease burden related to diabetes is increasing greatly, particularly in older subjects. A more comprehensive approach towards the assessment and management of diabetes’ comorbidities is necessary. An integrated clinical pathway (ICP) is necessary to achieve this goal as well as the implementation of its use in daily practice.
