Purpose of review To describe the epidemiology, prognostication, and treatment of out- and in-hospital cardiac arrest (OHCA and IHCA) in elderly patients. Recent findings Elderly patients undergoing cardiac arrest (CA) challenge the appropriateness of attempting cardiopulmonary resuscitation (CPR). Current literature suggests that factors traditionally associated with survival to hospital discharge and neurologically intact survival after CA cardiac arrest in general (e.g. presenting ryhthm, bystander CPR, targeted temperature management) may not be similarly favorable in elderly patients. Ultraviolet B Irradiation Modulates the Immune System of Fish (Rutilus rutilus, Cyprinidae) II: Blood

The effects of a single dose of ultraviolet B (UVB) radiation (0.4 J/cm2) on immunological functions by blood leukocytes and on hematological parameters was studied in roach (Rutilus rutilus), a teleostean fish. The respiratory burst of phorbol 12-myristate 13-acetate stimulated whole blood phagocytes increased significantly after UVB irradiation but spontaneous cytotoxicity of blood leukocytes toward 51chromium-labeled K562 target cells was not markedly altered. Resting sympatho-vagal balance is related to 10 km running performance in master endurance athletes

Relationships between heart rate recovery after exercise (HRR, baseline heart rate variability measures (HRV), and time to perform a 10Km running trial (t10Km) were evaluated in "master" athletes of endurance to assess whether the measured indexes may be useful for monitoring the training status of the athletes. Ten “master” athletes of endurance, aged 40-60 years, were recruited. After baseline measures of HRV, the athletes performed a graded maximal test on treadmill and HRR was measured at 1 and 2 minutes from recovery. Randomized, Double-Blind, Placebo-Controlled Study to Assess the Efficacy of Nonopioid Analgesics on Pain following Arthroscopic Knee Surgery

In a randomized, double-blind trial, the efficacy of nonopioid analgesics on postoperative piritramide consumption was compared for pain relief during the first 24 h in patients recovering from arthroscopic knee surgery. Methods. 120 patients were treated with normal saline and/or one of the nonopioid analgesics (parecoxib, metamizole, paracetamol) in addition to piritramide using the PCA pump. This paper analyzes problems that result in an alteration of mood, particularly in the sadness found in people with a serious physical illness at the end of life. The problems and implications of an faulty diagnosis, the confusions associated with a diagnosis of depression, and the importance of differen-tiation between “normal” mood reactions to illness, and pathological reactions, are analyzed. Several studies are reviewed and analyzed that propose the incorporation of a new syndrome to the traditional psychiatric classifications: demoralization. Finally the implications, advantages, and inconveniences of including a demoralization syndrome are discussed.
