Roflumilast N-oxide combined with PI3Kδ inhibitor improves the phenotype of early-onset COPD

2016

Background: Severe, early-onset COPD (Chronic Obstructive Pulmonary Disease) is characterized by a rapid decline in lung function at an early age with neutrophil over-activation. Roflumilast is approved as treatment for moderate and severe COPD at risk of exacerbation as add on therapy. Recent evidence indicates that the combination of PDE4 and PI3Kδ inhibitors show synergic anti-inflammatory properties. Objectives: To explore the effects from adding a selective PI3Kδ inhibitor to roflumilast N-oxide (RNO) in neutrophils isolated from peripheral blood of severe, early-onset COPD patients in in vitro models. Methods: Neutrophils were isolated from peripheral blood of 20 severe, early-onset C…

A Statistical Study to Analyze the Impact of External Weather Change on Chronic Pulmonary Infection in South Norway with Machine Learning Algorithms

2021

In this paper, we analyzed the holistic impact of external weather on chronic pulmonary infection in the Agder region with traditional machine learning algorithms. Millions of people are diagnosed with Chronic Obstructive Pulmonary Disease (COPD). Our study is dedicated in the Agder region, the Southern part of Norway. Norway has four seasons – winter (December-February), late winter/spring (March-May), Summer (June-August), and Autumn (September-November) in a year with average annual temperature approx. 7.5 °C | 45.5 °F and an annual rainfall of 1260 mm or 49.6 in. in Kristiansand. As predicted by the World Health Organization (WHO), in 2016, Norway suffered from 8% mortality due to c(1)h…

Sedation and analgesia during noninvasive ventilation (NIV)

2017

The use of noninvasive ventilation (NIV) has increased significantly in patients with acute respiratory failure (ARF) in order to prevent tracheal intubation and its complications. Unfortunately, NIV failure represents a frequent event, with rates that in some cases reach 40%. Mask intolerance, agitation, and delirium may lead to NIV failure, thus requiring endotracheal intubation. NIV failure rates are higher in patients without chronic obstructive pulmonary disease (COPD), and, when used in acute hypoxemic failure, its failure is associated with an increased mortality rate. The practice of sedation during NIV could be a valuable option for patients at risk of intubation. Sedation may decr…

15-years mortality in asthma-COPD overlap syndrome (ACOS) elderly population

2015

Background: The coexistence of asthma and chronic obstructive pulmonary disease (Asthma-COPD Overlap Syndrome: ACOS) is increasingly recognized. Data about the prevalence of ACOS in the elderly and its prognostic implications are unknown. Aim: To evaluate the long-term mortality of ACOS in an elderly population. Methods: 1970 subjects, aged >65 yrs, were enrolled in the population-based SA.R.A. Study. ACOS was defined as: not fully reversible airflow limitation + a) history of asthma or b) very positive bronchodilator test (increase in FEV1 ≥15% and ≥400 ml) + atopy + weezing. The prevalence of ACOS and 15-years mortality rates were assessed in 1063 subjects with complete clinical, lung fun…

Late Breaking Abstract - CRAC channel inhibition by RP3128 triggers potent anti-inflammatory effects in COPD or asthmatic patient-derived primary cel…

2018

Introduction: Calcium release activated calcium channels inhibitors have a potent role in treatment of autoimmune disorders. RP3128 is a novel and potent inhibitor of Calcium Release Activated Calcium (CRAC) channel activity with demonstrated efficacy in several models of immune-inflammatory disorders. The objective of this study was to assess the effect of RP3128 and combinations in attenuating inflammatory responses associated with respiratory diseases. Methods: Inhibition of antigen-induced cytokine release in primary T-lymphocytes was estimated. Peripheral blood neutrophils or mononuclear cells were isolated from COPD or asthmatic patients, pre-incubated with RP3128 for 0.5 h, and stimu…

Deep Neural Networks for Prediction of Exacerbations of Patients with Chronic Obstructive Pulmonary Disease

2018

Chronic Obstructive Pulmonary Disease (COPD) patients need help in daily life situations as they are burdened with frequent risks of acute exacerbation and loss of control. An automated monitoring system could lead to timely treatments and avoid unnecessary hospital (re-)admissions and home visits by doctors or nurses. Therefore we present a Deep Artificial Neural Networks for approach prediction of exacerbations, particularly Feed-Forward Neural Networks (FFNN) for classification of COPD patients category and Long Short-Term Memory (LSTM), for early prediction of COPD exacerbations and subsequent triage. The FFNN and LSTM models are trained on data collected from remote monitoring of 94 pa…

Effect of tiotropium/olodaterol combination therapy on long-term heart rate and blood pressure in COPD patients

2018

Introduction: Cardiovascular (CV) comorbidities are common in COPD, and associated with poor prognosis. LABAs and LAMAs are established COPD treatments whose pharmacology would suggest the potential to increase heart rate (HR) and impact blood pressure (BP). However, previous studies indicate that HR and BP are not negatively influenced by tiotropium (Tio) or olodaterol (Olo) monotherapy. Aims: To determine the effect of dual bronchodilation with Tio/Olo (T/O) on HR and BP. Methods: The 52-week, Phase III TONADO® studies (NCT01431274/NCT01431287) evaluated T/O 5/5 µg, Tio 5 µg or Olo 5 µg, via the Respimat® inhaler, in GOLD 2–4 COPD patients. In this post hoc analysis, long-term changes fro…

Inhaled steroids and prevalence of osteoporosis in chronic obstructive pulmonary disease: Data from the DACCORD cohort

2020

Introduction: Osteoporosis is a frequent COPD comorbidity with age, female gender, smoking, immobility and the use of oral steroids (OCS) as common risk factors. The role of inhaled steroids (ICS) is still under debate. Methods: Baseline data from the real life cohort DACCORD were analyzed. The diagnosis of osteoporosis was based on patients’ reports of physician-based diagnoses. Associations of ICS exposure at time of inclusion and osteoporosis were examined by group comparisons and multivariable logistic regression analyses with age, gender, smoking, exacerbation history (EH) and FEV1% as covariates. To avoid interactions with other steroid containing medications, exposure to any corticos…

Late Breaking Abstract - Relationship between clinical features and mortality in a cohort of COPD patients

2021

Background: Chronic obstructive pulmonary disease (COPD) is a progressive disease with poor outcomes, including a high mortality. GULP is a multicenter Italian study that described clinical characteristics, exacerbation rates and survival of a cohort of COPD patients, as part of AstraZeneca’s AvoidEX program. Aims and objectives: To evaluate whether the clinical profile of subjects enrolled in the GULP study predicts mortality on a period of 3 years. Methods: Post-hoc analysis of an observational, multicenter, retrospective study. Patients were stratified according to survival status: alive (A) and dead (D). Clinical characteristics, comorbidities and exacerbation rates were reported in the…
