2006

Aim. Although with different results, dialysis and kidney transplantation represent two effective strategies in treating chronic uraemia. Our study compares the psychological aspects of two categories of patients: on one hand those who faced kidney transplantation and have been in dialysis, on the other hand the non complaint patients treated with these therapies. Methods. We used a test for the analysis of the personality (MMPI2) and the COPE, that tests the abilities to coping used by the patient in certain conditions that can be perceived us stressful or, in any case, as unusual. With both MMPI2 and COPE we tested 120 patients that have faced haemodialysis and 50 that have been subject t…

Acute appendicitis and endometriosis: Retrospective analysis in emergency setting

2013

Introduction. The aim of this study is to evaluate the incidence of appendiceal and pelvic endometriosis in a population with diagnosis of acute appendicitis in an emergency setting. Materials and methods. We carried out a retrospective study in the period between January 2010 and October 2013. We performed 429 appendectomy including 233 in female subjects (54.3%). In all patients of childbearing age, we did β-hCG urine test to rule out the presence of an extra-uterine pregnancy. Results. 127 of 233 patients received a laparoscopic approach (54.5%). The median age was 29 years-old (range 17-68). The diagnosis of appendiceal and/or pelvic endometriosis was performed in 10 patients (4.3%): 9 …

Surgical palliation for malignant obstructive jaundice:our experience

2006

BACKGROUND): The prognosis of patlcnts with priimsrv bilian and pancreatic mahgnances is poor. At the time of diagnosis, approxiiuately $0°’ of paticnts are found to hae an nnresectable tumour, because of local sprcad or metastatic disease. ‘lherefore, most patients wilI undergo j,alhative treatment, which is amcd at the ilnprovetnent ofthe quality oflife and the prevention of the svmptoms. This study report personal experience and deserihes the bcst evidence in recent literature. PA’IIENThAND METIIO1)S: 20 paticnts with malignant obstructie jaundice for unresectable hihar or pancreatic neoplasm underwent palliative surgieal treatment. Enteral nutrition: Our experience with percutaneous endoscopic gastrostomy (PEG) and revision of literature

2012

Enteral nutrition (EN), as parenteral nutrition (PN), can be used in cases of patients whose medical conditions prevent the intake of food by mouth; unlike PN, EN keeps the functionality of the digestive tract and it makes home management of patients easier. However, the experience and literature have documented a number of serious complications, fortunately rare, which depend on the methods used in EN realization. We report in this paper our experience in 44 cases of percutaneous endoscopic gastrostomy (PEG), concluding that it is a safe and complications-free procedure. We believe that a nutritional intervention is indicated when, improving nutritional status, patients can obtain a better…

Port-site hernia following laparoscopic cholecystectomy: personal experience and literature review

2015

Background The purpose of our study is to define the real incidence of port-site hernias (PSH) following laparoscopic cholecystectomy (LC) through a long-term follow-up and to evaluate the significance of several technical and patient-related factors. Materials and Methods A total of 513 consecutive patients who underwent LC at Section of General and Thoracic Surgery of the University Hospital of Palermo in the period between January 2008 and January 2012 were retrospectively examined. The pneumoperitoneum at 12 mmHg was always established with Hasson’s technique at the umbilical site. Surgery was performed using the French technique. Closure of the fascial defect was performed only at the …

Adrenal cavernous hemangioma: which correct decision making process?

2016

Introduction: Cavernous hemangioma of the adrenal gland is a rare benign tumor characterized by the presence of blood-fil- led, dilated vascular spaces. These adrenal masses are usually non-functioning and the patients have no symptoms so the diagnosis is incidental. Methods: We performed a systematic literature review for all articles published until April 2015. The initial search identified 98 publications. We considered some characteristics: the mean age of the patients at diagnosis was 59 years (range 19 - 84); there were approximately 1.7 times more female patients than male patients; mean diameter of the lesions was 10.3 cm (range 2 - 25). Surgical treatment was more often open with m…

MANAGEMENT OF LEIOMYOMA OF THE TRANSVERSE COLON: CASE REPORT

2013

Colonic leiomyoma is a mesenchymal tumor that arises from the muscularis mucosae or muscularis propria and is composed of well-differentiated smooth muscle cells with no atypia. It is often incidentally found since its growth affects the submucosal layer and the lesion is covered with normal epithelium. Endoscopic ultrasonography is recommended to define the grade of infiltration of the tumor and eventually lymph node involvement. Histological examination is critical to establish the nature of the tumor and its behaviour. In the case of a voluminous tumor surgical treatment is needed. we report case of a patient that underwent colonoscopy showing the presence of a neoformation at 70 cm from…

LONG-TERM RESULTS AFTER ENDOSCOPIC DILATION OF POST-OPERATIVE COLO-COLONIC ANASTOMOTIC STENOSES. OUR EXPERIENCE IN 42 PATIENTS.

2013

The aim of this study is to evaluate long-term complications and patient's quality of life after the endoscopic treatment of benign anastomotic colo-colonic strictures. From January 2000 to November 2008,, 42 patients who had undergone surgery for colorectal cancer were endoscopically treated for a postoperative symptomatic stricture. The dilation were performed using a 20-30 mm pneumatic dilator. The clinical results were classified in relation to the abdominal symptomatology reported by the patients, and were evaluated in the short-term (one-week) and long-term (mean follow-up.36 months)period. Results: 15 patients underwent a total of 22 dilating sessions; 9 patients had a single dilatin…

COULD THE ENDOSCOPIC RESECTION OF A LARGE RECTAL LEIOMYOMA BE AN EFFETCIVE AND SAFE TECHNIQUE?

2016

Summary. Rectal leiomyomas are a rare conditions, with low reported incidence in literature and constitute about 0.1% of rectal tumours; in fact rectal leiomyomas occur in approximately 1 out of 2000-3000 rectal tumors. We report on a patient with a 3 cm semi-pedunculated colonic leiomyoma, which was successfully removed by endoscopic polypectomy after normal saline-epinephrine submucosal injection. When we encounter a tumor during a colonoscopic examination, we usually evaluate the tumor carefully and perform an endoscopic resection when we judge it is appropriate. When a symptomatic smooth muscle tumors smaller than 2 cm are incidentally found on colonoscopy, surgical resection is unneces…
