Giant right atrial myxoma: A rare cause of cardiovascular collapse

We report the case of a patient with a giant right atrial myxoma that remained clinically silent until it almost completely obliterated the right atrium, prolapsed into the right ventricle and obstructed the tricuspid valve inflow. This case illustrates the importance of rapid surgical intervention in the setting of acute heart failure caused by tumor masses obliterating heart valves or cardiac chambers.

Quantitative assessment of regularity and synchronization of intracardiac recordings during human atrial fibrillation

This study proposes the morphology-based evaluation of the regularity (R) and the synchronization (S) of intra-atrial electrograms acquired during atrial fibrillation (AF). R is defined as the degree of repetitiveness over time of the shapes of the activation waves detected in single atrial recordings. S accounts for the simultaneous presence of morphologically similar activation waves in two atrial electrograms, and for the dispersion of the propagation delays between the two sites. OBJECTIVE To find out whether pulmonary thromboendarterectomy (PTE) can achieve lasting reduction of pulmonary vascular resistance in patients with pulmonary arterial hypertension due to chronic thromboembolism. PATIENTS AND METHODS 45 patients (25 women, 20 men; mean age 45 +/- 24 [19-67] years) were re-investigated a mean of 21 (13-32) months after successful PTE. Two patients had then been in New York Heart Association (NYHA) stage II, 26 in stage III, and 17 in stage IV. In their editorial, Buck and Erbel1 raise the important question in which settings application of contrast agents to an echocardiographic examination is appropriate. There is no doubt that the beauty of echocardiography relates to its ease of use. We should acknowledge that highest accuracy in the determination of left ventricular function is not necessary in most patients referred for an echocardiographic evaluation. OBJECTIVES To define the use of cineventriculography, cardiac magnetic resonance imaging (cMRI), and unenhanced and contrast-enhanced echocardiography for detection of left ventricular (LV) regional wall motion abnormalities (RWMA). BACKGROUND Detection of RWMA is integral to the evaluation of LV function. METHODS In 100 patients, cineventriculography and unenhanced and contrast-enhanced echocardiography were performed. Three-dimensional transesophageal echocardiography in the guide of cardiac mass biopsy

Cardiac tumors are rare. Cardiovascular imaging is more important in the differential diagnosis of cardiac masses but no current noninvasive diagnostic tool has the ability to absolutely diagnose cardiac tumors. In effect cardiac biopsy remains the gold standard in the differential diagnosis. In our case we show the advantages of three-dimensional (3D) transesophageal echocardiography as a guide for cardiac biopsy. We believe that 3D TEE is ready to be used in the guide of cardiac mass biopsy. Once interventionalists become confident with 3D imaging, this technique should be used routinely during these procedures.

Congenitally corrected transposition of great vessels: MRI and echocardiographic appearance

This paper presents the case of a 36-year-old male with congenitally corrected transposition of great vessels (CTGV). This is a rare cardiac anomaly characterized by atrioventricular and ventriculoarterial discordance, occuriing in less than 1% of all congenital heart diseases. This was diagnosed by echocardiography and MRI. The MRI findings demonstrated cardiac abnormality.
