Three-dimensional parametric modeling of bicuspid aortopathy and comparison with computational flow predictions

2017

Bicuspid aortic valve (BAV)-associated ascending aneurysmal aortopathy (namely "bicuspid aortopathy") is a heterogeneous disease making surgeon predictions particularly challenging. Computational flow analysis can be used to evaluate the BAV-related hemodynamic disturbances, which likely lead to aneurysm enlargement and progression. However, the anatomic reconstruction process is time consuming so that predicting hemodynamic and structural evolution by computational modeling is unfeasible in routine clinical practice. The aim of the study was to design and develop a parametric program for three-dimensional (3D) representations of aneurysmal aorta and different BAV phenotypes starting from s…

Antithrombotic therapies in aortic and peripheral arterial diseases in 2021: a consensus document from the ESC working group on aorta and peripheral …

2021

AbstractThe aim of this collaborative document is to provide an update for clinicians on best antithrombotic strategies in patients with aortic and/or peripheral arterial diseases. Antithrombotic therapy is a pillar of optimal medical treatment for these patients at very high cardiovascular risk. While the number of trials on antithrombotic therapies in patients with aortic or peripheral arterial diseases is substantially smaller than for those with coronary artery disease, recent evidence deserves to be incorporated into clinical practice. In the absence of specific indications for chronic oral anticoagulation due to concomitant cardiovascular disease, a single antiplatelet agent is the ba…

Treat-to-target versus dose-adapted statin treatment of cholesterol to reduce cardiovascular risk

2015

Clinical guidelines should be based on the best available evidence and are of great importance for patient care and disease prevention. In this respect, the 2013 American College of Cardiology/American Heart Association report is highly appreciated and well-recognized. The report included critical questions concerning hypercholesterolaemia, but its translation into a clinical guideline initiated intense debate worldwide because of the recommendation to switch from a treat-to-target approach for low-density-lipoprotein-cholesterol to a statin dose-based strategy.

¿De qué nos sirve evaluar las tendencias del perfil glucémico ambulatorio?

2014

The ambulatory glucose profile is an instrument used to detect blood glucose patterns from patient results by recording blood glucose results or, more usually, by continuous glucose monitoring. This instrument analyzes the changes in glucose levels obtained over a number of days, and expresses the data, depending on the time of the day, using the median and the 10th, 25th, 75th and 90th percentiles. It is very useful for identifying glycemic patterns over the day, to quantify blood glucose variability, and to classify hypoglycemia risk according to time. This should be complemented with the analysis of individual blood glucose data, diet, and exercise, to detect potential causes of variabil…

Microvascular blood flow changes of the abductor pollicis brevis muscle during sustained static exercise

2021

A practical assessment of the general health and microvascular function of the palm muscle, abductor pollicis brevis (APB), is important for the diagnosis of different conditions. In this study, we have developed a protocol and a probe to study microvascular blood flow using near-infrared diffuse correlation spectroscopy (DCS) in APB during and after thumb abduction at 55% of maximum voluntary contraction (MVC). Near-infrared time resolved spectroscopy (TRS) was also used to characterize the baseline optical and hemodynamic properties. Thirteen (n=13) subjects were enrolled and subdivided in low MVC (N=6, MVC<2.3 kg) and high MVC (N=7, MVC≥2.3 kg) groups. After ruling out significant cha…

Changes in motor unit characteristics after eccentric elbow flexor exercise

2010

Morphological evidence suggests that fast-twitch fibers are prone to disruption of their membrane structures by eccentric exercise. However, it is unclear how this is reflected in the discharge rate and action potential propagation of individual motor units, especially at high contraction levels. High-density surface electromyograms were recorded from biceps brachii muscle and decomposed to individual motor unit action potentials at isometric contraction levels between 10% and 75% of maximal voluntary contraction (MVC) before intermittent maximal elbow flexor eccentric exercise, and two hours (2H), two days (2D) and four days (4D) post-exercise. Maximal voluntary force decreased by 21.3±5.6…

Soleus and lateral gastrocnemius H-reflexes during standing with unstable footwear

2015

Introduction: Unstable footwear has been shown to increase lower extremity muscle activity, but the reflex response to perturbations induced by this intervention is unknown. Methods: Twenty healthy subjects stood in stable and unstable footwear conditions (presented randomly) while H-reflex amplitude and background muscle activity were measured in the soleus and lateral gastrocnemius (LG) muscles. Results: Wearing unstable footwear resulted in larger H-reflexes (normalized to the maximal M-wave) for the LG (+12%; P = 0.025), but not for the soleus (+4%; P > 0.05). Background activity of both muscles was significantly higher in the unstable condition. Conclusions: The H-reflex facilitation o…
