ICP- and IOP-Effects of Deliberate Hypotension Using Urapidil

1989

Deliberate hypotension is an accepted method to facilitate certain neurosurgical and intraocular procedures. The rationale is to reduce transmural pressure on arterial vessels and thus to facilitate preparation of cerebrovascular aneurysms and malfunctions — or in ophthalmology to reduce the risk of expulsive bleeding (Jantzen and Earnshaw 1988).

Evaluation of nitroglycerin effect on remote photoplethysmogram waveform acquired at green and near infra-red illumination

2017

Assessment of skin microcirculation provides diagnostically valuable information during the early stages of pathologies. The simple, cost-effective and intrusive alternative to existing circulation assessment methods is remote photoplethysmography (rPPG). The objective of the present pilot study was to reveal an effect on sublingual administration of 1 mg nitroglycerin on systemic hemodynamic parameters and rPPG waveforms, at 810 nm and 530nm illumination. The protocol comprised 3 minutes of baseline recording, 15 minutes recording of NTG effect, 2 minutes of arterial occlusion and the following 3 min reactive hyperemia. Two PPG signals were acquired from glabrous skin of the middle finger …

Hiperaldosteronismo primario por hiperplasia suprarrenal unilateral con resolución quirúrgica

2016

Unilateral adrenal hyperplasia is a rare cause of primary hyperaldosteronism (around a 3%) that has surgical treatment. A case of a patient with hypertension resistant to conventional therapy in treatment with 7 drugs who presented with primary hyperaldosteronism due to unilateral adrenal hyperplasia is presented. A left adrenalectomy was performed, and the patient had a good clinical response, with no need of any drug after 2 years of surgery. Unilateral adrenal hyperplasia is a different entity and it is not an asymmetric variant of the bilateral adrenal hyperplasia. In the study of patients with primary hyperaldosteronism and imaging tests with absence of adenoma is a diagnosis that must…

Indicaciones actuales de revascularización

2012

Randomized controlled trials in patients with chronic ischemic heart disease and stable angina indicate that the majority of cases can be managed by optimum medical treatment. The exceptions are patients with conditions that could benefit from revascularization, such as recurrent angina, extensive inducible ischemia or complex coronary anatomy. However, since studies have traditionally excluded patients with significant comorbidity, it is not clear which treatment approach would be best in these circumstances. Although patients with comorbidities are usually treated conservatively in routine clinical practice, some recent studies in elderly patients have raised the possibility that this sub…

Cardiorenal Fat: A Cardiovascular Risk Factor With Implications in Chronic Kidney Disease.

2021

There is a growing interest in the potential role of adipose tissues in cardiac and renal pathophysiology, and determining the mechanisms by which fat compartments around the heart and kidneys influence cardiovascular disease is of clinical importance in both general and high-risk populations. Epicardial fat and perirenal fat have been associated with adverse outcomes in chronic kidney disease (CKD) patients. Epicardial fat is a rich source of free fatty acids and is capable of secreting inflammatory and pro-atherogenic cytokines that promote atherosclerosis through a local paracrine effect. Recent evidence has demonstrated that perirenal fat has a closer correlation with kidney diseases th…

Mediterranean diet and antihypertensive drug use: a randomized controlled trial

2021

Objective To examine in older individuals at high cardiovascular risk whether following a Mediterranean diet decreased the necessity of antihypertensive drugs and modulated their associated cardiovascular risk. Methods In the PREvencion con DIeta MEDiterranea study, we assessed whether volunteers randomly allocated to an intervention with a Mediterranean diet enriched with extra-virgin olive oil or nuts (relative to a low-fat control diet) disclosed differences in the risk of: initiating antihypertensive medication in nonusers at baseline (n = 2188); and escalating therapy in participants using one, two, or three drugs at baseline (n = 2361, n = 1579, and n = 554, respectively). We also ass…

Postprandial metabolism: from research to clinical practice.

2013

Over the last decade, the importance of postprandial metabolism has increased, given that it is the physiological state of humans in modern society. Moreover, postprandial lipemia is considered as a key player in the development of the most important cardiometabolic diseases. In this regard, postprandial lipemia has become more important, since it has been demonstrated that nonfasting triglycerides (TGs) are independent predictors of the risk of cardio vascular disease (CVD) [1,2]. Interestingly, the postprandial phase has been associated with increased inflammation and oxidation, which influences vascular function through a permanent endothelial aggression by atherogenic lipoprotein. TGs a…
