Platelet Function Changes In Acute Myocardial Infarction

1981

Whether the thrombotic component of myocardial infarction is primary or secondary in a given patient, platelet function alterations can influence many mechanism from which depends if the thrombotic lesion grows or sends platelet emboli to the smaller myocardial vessels. Recently in some cases of infarction, coronary artery spasm has been demonstrated angiographically; thromboxanes, vasoconstrictive and platelet-aggregating substances, are released by platelets during myocardial ischemia. The local release of these substances may modify the myocardial cell viability and regional blood flow. The aim of the present study was to investigate changes in platelet function in relation to the time in…

A Normal Fractional and Coronary Flow Reserve

2016

Fractional flow reserve (FFR) and coronary flow reserve (CFR) provide different and complementary information on vascular physiology. The two tests provide information, respectively, on the epicardial and total vascular resistances. Typically, a FFR <0.75 is also associated with an impaired CFR (<2); exceptions, however, exist.

A Complex Combination of Microvascular and Macrovascular Diseases

2016

When FFR is larger than 0.80, a PCI is not justified. Transient microvascular dysfunction may, however, be associated with impaired hyperemia and falsely negative FFR. This case also emphasizes the dynamic nature of microvascular disease, and provides an example of both “typical” syndrome X and “less conventional” syndrome slow flow associated with increased peripheral resistances (see Chap. 35).

P5017Factors associated with a negative D-dimer test in patients diagnosed with acute symptomatic pulmonary embolism

2019

Abstract Background The recommended diagnostic strategy for suspected acute pulmonary embolism (PE) combines the assessment of pre-test probability, D-dimer level, and -if indicated- computed tomography pulmonary angiography. Purpose To evaluate the frequency and potential explanations for negative D-dimer tests in patients diagnosed with acute PE. Methods The multicentre Follow-up of Acute Pulmonary Embolism (FOCUS) cohort study prospectively enrolled 1,100 consecutive patients diagnosed with acute symptomatic PE; two-year follow-up is ongoing. The items of the Simplified revised Geneva Score and the D-dimer levels at diagnosis have been prospectively collected, but they did not necessaril…
