Clinical outcome of critically ill, not fully recompensated, patients undergoing MitraClip therapy

Aims As periprocedural risk is low, MitraClip implantation is often performed in critically ill, not fully recompensated patients, who are in NYHA functional class IV at the time of the procedure, to accelerate convalescence. We herein sought to evaluate the procedural and 30-day outcome of this patient group. Methods and results A total of 803 patients undergoing MitraClip implantation were included in the German Mitral Valve Registry, and 30-day outcomes were prospectively assessed. With advances in imaging technology and our understanding of the role of inflammation in atherosclerosis, the macrophage appears to be an excellent target for imaging the progression of disease. In addition to imaging the macrophage with only 1 modality, contrast agents can be created that can be imaged with multiple modalities. This seems extremely attractive, as lesion morphology and characteristics can be determined with modalities that provide high picture resolution, such as CT or MRI, whereas macrophage quantity can be accurately determined through the creation of a radiolabeled contrast agent such as FDG via PET. Interaction between muscle stiffness and stretch reflex sensitivity after long-term stretch-shortening cycle exercise

The short latency stretch-reflex component (M1) and its interactions with muscle stiffness and with muscle performance were investigated before and after long-term stretch-shortening cycle (SSC) exercise. Dramatic fatigue induced reduction in maximal SSC performance capability, and electromyographic activity was accompanied by a consistent decrease in the M1 reflex component and eccentric peak stiffness of the muscle. It can be suggested, therefore, that the decreased muscle performance is not simply a direct effect of central or peripheral fatigue, but is partly due to impairment of the ability to utilize stiffness-related elastic energy.

Quantitative assessment of regularity and synchronization of intracardiac recordings during human atrial fibrillation

This study proposes the morphology-based evaluation of the regularity (R) and the synchronization (S) of intra-atrial electrograms acquired during atrial fibrillation (AF). R is defined as the degree of repetitiveness over time of the shapes of the activation waves detected in single atrial recordings. S accounts for the simultaneous presence of morphologically similar activation waves in two atrial electrograms, and for the dispersion of the propagation delays between the two sites.
