The cementoblastoma is a rare odontogenic tumor occurring in the mandibular molar and premolar of the patients in the second and third decades of life. Despite its typical benign behavior, this tumor may promote local destruction by perforating the cortical bone and displacing the mandibular canal. This case report shows a 31-year-old man with an aggressive cementoblastoma attached to the apex of the mandibular first molar. A case of orofacial pain and inferior alveolar nerve (IAN) paraesthesia after extrusion of endodontic sealer within the mandibular canal treated with prednisone and pregabalin is described. A 36-year-old woman underwent root canal treatment of the mandibular second right premolar tooth. Post-operative panoramic radiograph revealed the presence of radiopaque canal sealer in the mandibular canal. Damage to IAN consecutive to extrusion of endodontic sealer was diagnosed. Non-surgical management was decided, including: 1 mg/kg/day prednisone 2 times/day, once-daily regimen, and 150 mg/day pregabalin, two doses per day, monitoring the progress with periodic follow-up visits. The treatment is recommended to be indicated by a physician, depending on each patient and on its characteristics. The management of Class III malocclusion is one of the greatest challenges of orthodontics. Current treatments offer the possibility of using direct skeletal anchorage to improve clinical outcomes. This case shows the results of using a Hyrax hybrid palatal anchorage, Alt-RAMEC (Alternate Rapid Maxillary Expansion and Contraction protocol) and a facemask to treat a maxillary hypoplasia Class III malocclusion in a Latin-American patient. The appliance design and the protocol used are widely described.
