A proposal for an evidence-based model of the screening for the colorectal carcinoma in an Italian setting

2011

Introduction. The aim of this paper is to improve the feasibility perception of policymakers, health care workers and target population about the cost-effectiveness of the implementation of colorectal screening as Public Health strategy. Methods. Retrospective study by application of a three-step model designed for a local setting in Sicily (Palermo and its Province) in order to distribute Fecal Occult Blood Tests (FOBTs), offer colonoscopy and surgery, by district allocation of pharmacies, public digestive endoscopic centres and oncologic and general surgery units. Mean adherence to consolidated colorectal screening programs in Italy was applied in order to evaluate the feasibility of an o…

Interleukin-17-positive mast cells influence outcomes from BCG for patients with CIS: Data from a comprehensive characterisation of the immune microe…

2017

The tumour immune microenvironment is considered to influence cancer behaviour and outcome. Using a panel of markers for innate and adaptive immune cells we set out to characterise and understand the bladder tumour microenvironment of 114 patients from a prospective multicentre cohort of newly-diagnosed bladder cancer patients, followed-up for 4.33±1.71 years. We found IL-17-positive cells were significantly increased in primary and concomitant carcinoma in situ (CIS), p<0.0001, a highly malignant lesion which is the most significant single risk factor for disease progression. Further characterisation of the tumour immunophenotype identified IL-17+ cells as predominantly mast cells rather t…

Advances in endoscopic imaging of colorectal neoplasia.

2010

Colon cancer screening is arguably the most important activity performed by gastroenterologists. Recent decreases in rates of death from colorectal cancer indicate that screening methods such as colonoscopy have a positive impact. There is still room for improvement, however, particularly in prevention of right-sided colon cancer. Practice issues, such as making colonoscopy more comfortable, safer, and less costly, are keys to continued success in cancer prevention. Colonoscopy techniques, technologies, and quality control measures have advanced to improve detection, classification, and removal of early neoplasias. In particular, slow, careful inspection of the colon by gastroenterologists …

Push-and-pull enteroscopy using a single-balloon technique for difficult colonoscopy.

2006

Background and study aims Several factors predict difficult or incomplete colonoscopy with a standard colonoscope, including female gender, low body mass index, extensive diverticulosis, and prior pelvic surgery. Patients and methods A push-and-pull enteroscopy device and a single-balloon technique was used in 14 patients (six men, eight women; mean age 62 +/- 15 years) after failure of ileocolonoscopy or endoscopic therapy using either a standard or a pediatric colonoscope. The original ileocolonoscopy had been performed to investigate colon polyps (n = 6), an obscure inflammatory process in the ileocecal region (n = 6), or iron deficiency anemia with a positive fecal occult stool test (n …

Angiogenesis, immune system and growth factors: new targets in colorectal cancer therapy.

2005

Colorectal cancer is the second most common malignant human neoplasia. Over recent years, many efforts have been performed in order to develop and improve therapeutic protocols, and many advances have been accomplished in both the field of adjuvant and palliative therapy. Most of the chemotherapic agents currently used in the clinical setting are the products of decades of research aimed at inhibiting the uncontrolled growth of dysplastic cells. However, new frontiers in this field have recently been opened, with the identification of key molecules involved in physiologic mechanisms that are of fundamental importance for cancer development and progression. Tumor-induced angiogenesis, the ca…

Colorectal cancer defeating? Challenge accepted!

2013

Colorectal tumours are actually considered as aberrant organs, within it is possible to notice a different stage of cell growth and differentiation. Their origin is reported to arise from a subpopulation of tumour cells endowed with, just like the healthy stem cells, self-renewal and aberrant multi-lineage differentiation capacity likely to be called colorectal cancer stem cells (CCSCs). Cancer stem cells (CSCs) fate, since their origin, reflects the influences from their microenvironment (or niche) both in the maintenance of stemness, in promoting their differentiation, and in inducing epithelial-mesenchymal transition, responsible of CSCs dissemination and subsequent formation of metastat…

Estimation of National Colorectal-Cancer Incidence Using Claims Databases

2012

Background. The aim of the study was to assess the accuracy of the colorectal-cancer incidence estimated from administrative data.Methods.We selected potential incident colorectal-cancer cases in 2004-2005 French administrative data, using two alternative algorithms. The first was based only on diagnostic and procedure codes, whereas the second considered the past history of the patient. Results of both methods were assessed against two corresponding local cancer registries, acting as “gold standards.” We then constructed a multivariable regression model to estimate the corrected total number of incident colorectal-cancer cases from the whole national administrative database.Results.The firs…

Supporting Autonomous Navigation of Visually Impaired People for Experiencing Cultural Heritage

2020

In this chapter, we present a system for indoor and outdoor localization and navigation to allow the low vision users in experiencing cultural heritage in autonomy. The system is based on the joint utilization of dead-reckoning and computer vision techniques on a smartphone-centric tracking system. The system is explicitly designed for visually impaired people, but it can be easily generalized to other users, and it is built under the assumption that special reference signals, such as colored tapes, painted lines, or tactile paving, are deployed in the environment for guiding visually impaired users along pre-defined paths. Differently from previous works on localization, which are focused …

Retrieval of Case 2 Water Quality Parameters with Machine Learning

2018

Water quality parameters are derived applying several machine learning regression methods on the Case2eXtreme dataset (C2X). The used data are based on Hydrolight in-water radiative transfer simulations at Sentinel-3 OLCI wavebands, and the application is done exclusively for absorbing waters with high concentrations of coloured dissolved organic matter (CDOM). The regression approaches are: regularized linear, random forest, Kernel ridge, Gaussian process and support vector regressors. The validation is made with and an independent simulation dataset. A comparison with the OLCI Neural Network Swarm (ONSS) is made as well. The best approached is applied to a sample scene and compared with t…

Retrieval of coloured dissolved organic matter with machine learning methods

2017

The coloured dissolved organic matter (CDOM) concentration is the standard measure of humic substance in natural waters. CDOM measurements by remote sensing is calculated using the absorption coefficient (a) at a certain wavelength (e.g. 440nm). This paper presents a comparison of four machine learning methods for the retrieval of CDOM from remote sensing signals: regularized linear regression (RLR), random forest (RF), kernel ridge regression (KRR) and Gaussian process regression (GPR). Results are compared with the established polynomial regression algorithms. RLR is revealed as the simplest and most efficient method, followed closely by its nonlinear counterpart KRR.
