Traumatic events can engender persistent excessive fear responses to trauma reminders that may return even after successful treatment. Extinction, the laboratory analog of behavior therapy, does not erase conditioned fear memories but generates competing, fear-inhibitory "extinction memories" that, however, are tied to the context in which extinction occurred. Accordingly, a dominance of fear over extinction memory expression--and, thus, return of fear--is often observed if extinguished fear stimuli are encountered outside the extinction (therapy) context. It was repeatedly demonstrated that a negative emotional context enhances memory for central details while impairing memory for peripheral information. This trade-off effect is assumed to result from attentional processes: a negative context seems to narrow attention to central information at the expense of more peripheral details, thus causing the differential effects in memory. One reason why people prefer to watch movies in cinemas may be the surround of the movie screen or its physically huge size. Palliative Sedation in Patients With Advanced Cancer Followed at Home: A Prospective Study

Abstract Context Home care programs in Italy. Objectives The aim of this study was to assess a protocol for palliative sedation (PS) performed at home. Methods A total of 219 patients were prospectively assessed to evaluate a PS protocol in patients with advanced cancer followed at home by two home care programs with different territorial facilities. The protocol was based on stepwise administration of midazolam. Results A total of 176 of the patients died at home, and PS was performed in 24 of these patients (13.6%). Younger patients received the procedure more frequently than older patients (P = 0.012). The principal reasons to start PS were agitated delirium (n = 20) and dyspnea (n = 4).…

Human event-related potentials (ERPs) and electro-oculograms (EOGs) were recorded in 14 subjects presented with spatially deviant tones in a series of standard tones. In separate sessions, they were instructed to read a book, to count the deviant tones, and to respond to the deviant tones by shifting the eyes towards them from the standard tone source. When reading a book, the mismatch negativity (MMN) of ERP, reflecting pre-attentive detection of acoustic changes, was elicited to the deviant tones at approximately 105-180 ms.
