BACKGROUND: The acute inflammatory response following mesh implantation has been often evaluated in vitro and in animal models. Because endurance exercise causes release of mediators and growth factors active on the bone marrow, we asked whether it might affect circulating hematopoietic progenitor cells (HPCs) in amateur runners [ n = 16, age: 41.8 ± 13.5 (SD) yr, training: 93.8 ± 31.8 km/wk] compared with sedentary controls ( n = 9, age: 39.4 ± 10.2 yr). Recent discoveries indicate that the major pathophysiology is a loss of hypocretin (orexin) producing neurons due to immunologically mediated degeneration. We studied in vitro cytokine production by peripheral blood mononuclear cells (PBMC) from patients with primary and recurrent hydatid disease when cells were incubated with mitogen (PHA) and antigen from hydatid cyst fluid (HCFAg); levels of specific IgE, IgG4 and eosinophil counts were also measured in sera. When specifically stimulated, PBMC from patients produced higher levels of IL-2 (P < 0.02), IFN-γ (P < 0.0028) and IL-5 (P < 0.01) than those from uninfected donors, whereas IL-10 levels were comparable. Complex regional pain syndrome (CRPS) develops after fracture. The acute CRPS phenotype resembles exaggerated inflammation, which is explained by local and systemic activation of a proinflammatory network including peptides and cytokines. Epidemiologic data suggest that inactivation of the peptidase angiotensin-converting enzyme in patients treated for hypertension increases the odds to develop CRPS. SUMMARY The pathogenic mechanisms underlying the development of autoimmune hepatitis (AIH) are still unclear. Since AIH is associated with the presence of various autoantibodics and certain HLA subtypes, it is likely that T and B cells play a major role in this disease. In this study we have determined the functional capacities of in vivo preactivated liver-infiltrating T cells (LTC) from patients with AIH. As controls we used LTC from patients with non-autoimmune hepatitis (non-AIH). Our results show that preactivated LTC from patients with AIH predominantly (190/255 clones) reside in the CD4+ population, whereas LTC in non-AIH are dominated by the CD8+ phenotype (148/254 clones).
