Blastic plasmacytoid dendritic cell neoplasm: genomics mark epigenetic dysregulation as a primary therapeutic target

Blastic Plasmacytoid Dendritic Cell Neoplasm is a rare and aggressive hematological malignancy currently lacking an effective therapy. To possibly identify genetic alterations useful for a new treatment design, we analyzed by whole-exome sequencing fourteen Blastic Plasmacytoid Dendritic Cell Neoplasm patients and the patient-derived CAL-1 cell line. The functional enrichment analysis of mutational data reported the epigenetic regulatory program as the most significantly undermined (P<.0001). The fundamental idea underlying this architecture is the use of a hierarchical communications structure called "logical namespace tree" and a structured set of control processes interconnected, called "logical sensors graph". The reaction of the lithium enolate of the 1,3-dioxolan-4-one derived from optically active (S)-mandelic acid and pivalaldehyde with α,β-unsaturated carbonyl compounds proceeds readily to give the corresponding Michael adducts in good yields and high diastereoselectivity. Subsequent basic hydrolysis of the acetal and oxidative decarboxylation of the α-hydroxyacid moiety provides chiral 2-substituted 1,4-dicarbonyl compounds with very high enantiomeric excesses.

AIM OF THE STUDY: To evaluate correlations between pre-operating epidemiological, clinical and diagnostic data and relapses concerning to patients operated on for benign thyroid nodules. MATERIAL AND METHODS: The AA. carried out a new ultrasonographic exam in 155 patients selected at random among 1012 treated by thyroid lobectomy between September 1976 and December 2002; 107 relapses were found (69%). Toxic adenoma and papillary thyroid carcinoma in a patient with Graves' disease.

A case of a very rare association of toxic adenoma and papillary carcinoma with Graves’ disease is presented. A 34-year-old woman developed Graves’ disease with mild ophthalmopathy. An ultrasound revealed diffuse thyroid enlargement with a hypoechoic pattern and a hypoechoic nodule with regular edges of 1.6 cm in diameter at the lower pole of the left lobe. A thyroid 131I scintiscan showed a diffuse and homogeneous 131I distribution. The 131I uptake (RAIU) was elevated.
