Serous Macular Detachment Associated with Dome-Shaped Macula and Tilted Disc

Introduction: An entirely new type of staphyloma has been recently described as dome-shaped macula (DSM). It is characterized by an abnormal convex macular contour within the concavity of a posterior staphyloma. We found DSM associated with serous macular detachment (SMD) and tilted disc in two consecutive cases. Case Reports: Case 1: A 37-year-old female presented to our department because of sudden onset blurred vision in her right eye (OD). The best-corrected visual acuity (BCVA) was 0.5 in both eyes. Purpose Cystoid macular edema may occur following scleral buckling and therefore deteriorate the visual outcome. Inflammation may be the major causative factor in the development of postoperative cystoid macular edema. This case demonstrates the effectiveness of a dexamethasone implant as a treatment after the onset of choroidal inflammation and cystoid macular edema 6 months following scleral buckling and having visual acuity restored. Since the original description by Goodwin in 1955, percutaneous nephrostomy (PNS) has assumed an important role in the management of obstructive uropathy. It was initially devised as an alternative to operative nephrostomy, but has now completely replaced the latter, for the following good reasons: 1. It can be done under local anesthesia. 2. It provides effective urinary drainage. 3. It carries an acceptable risk, with low morbidity and practically no mortality.

Probes of the subretinal fluid were obtained from 115 nonselected retinal detachments, operated on during the period from November 19, 1973 to December 20, 1974. All patients had no known endocrine abnormalities and had not ever received topical or systemic corticosteroid therapy. Subretinal fluid Cortisol was determined by competitive protein binding analysis. A statistically significant increase in mean subretinal fluid Cortisol concentration was found for the winter season (3.3 µg/100 ml), as compared with the summer months (1.5 µg/100 ml).
