Challenges in clinical analysis by infrared spectroscopy
2014
CT angiography for the assessment of EVAR complications : a pictorial review
2022
Aortic aneurysm; Blood vessel prosthesis implantation; Endovascular procedures Aneurisma aórtico; Implantación de prótesis de vasos sanguíneos; Procedimientos endovasculares Aneurisma aòrtic; Implantació de pròtesis de vasos sanguinis; Procediments endovasculars Endovascular aneurysm repair (EVAR) is a minimally invasive treatment proposed as an alternative to open repair in patients with abdominal aortic aneurysms. EVAR consists in a stent-graft placement within the aorta in order to exclude the aneurysm from arterial circulation and reduce the risk of rupture. Knowledge of the various types of devices is mandatory because some stents/grafts are more frequently associated with complication…
Wilson’s Disease: Facing the Challenge of Diagnosing a Rare Disease
2021
Wilson disease (WD) is a rare disorder caused by mutations in ATP7B, which leads to the defective biliary excretion of copper. The subsequent gradual accumulation of copper in different organs produces an extremely variable clinical picture, which comprises hepatic, neurological psychiatric, ophthalmological, and other disturbances. WD has a specific treatment, so that early diagnosis is crucial to avoid disease progression and its devastating consequences. The clinical diagnosis is based on the Leipzig score, which considers clinical, histological, biochemical, and genetic data. However, even patients with an initial WD diagnosis based on a high Leipzig score may harbor other conditions th…
Quando l’amnios si rompe troppo presto e… da solo
2022
Rupture of the amniochorionic membranes is a recognized risk factor in the second and third trimester for chorioamnionitis, sepsis and preterm delivery. The isolated rupture of amnion is conversely responsible for a malformation spectrum known as early amnion rupture sequence or ADAM (Amniotic Deformity, Adhesions, Mutilations) complex, characterized by asymmetrical congenital anomalies, including limb reduction and craniofacial defects, usually arising during the 2nd or 3rd trimester of gestation. The paper reports the case of a 4-month-old infant who was referred for genetic counselling in the suspicion of malformation syndrome with asymmetrical limb reduction defects. The present case un…

Clinical utility of HFABP in myocardial infarction
2018
Assessing chest pain patients presenting to the emergency area (EA) is still a clinical challenge, as acute myocardial infarction (AMI) diagnosis is not adjudicated in the majority of patients. New generation high sensitivity troponin assays (hs-cTn) still present some limitations, thus, novel biomarkers to early rule-in and rule- out myocardial infarction in chest pain patients presenting to the EA are sought after. Among all, heart- type fatty acid binding protein (h-FABP) has been largely investigated. Studies performed on HFABP in these patients present marked heterogeneity. However, it can be stated that HFABP is clearly not a reliable marker for AMI diagnosis, neither as a stand-alone…
Epidemiology of congenital diaphragmatic hernia in Europe: a register-based study.
2015
INTRODUCTION: Published prevalence rates of congenital diaphragmatic hernia (CDH) vary. This study aims to describe the epidemiology of CDH using data from high-quality, population-based registers belonging to the European Surveillance of Congenital Anomalies (EUROCAT).METHODS: Cases of CDH delivered between 1980 and 2009 notified to 31 EUROCAT registers formed the population-based case series. Prevalence over time was estimated using multilevel Poisson regression, and heterogeneity between registers was evaluated from the random component of the intercept.RESULTS: There were 3373 CDH cases reported among 12 155 491 registered births. Of 3131 singleton cases, 353 (10.4%) were associated wit…
Safety and immunomodulatory effects of three probiotic strains isolated from the feces of breast-fed infants in healthy adults: SETOPROB study.
2013
We previously described the isolation and characterization of three probiotic strains from the feces of exclusively breast-fed newborn infants: Lactobacillus paracasei CNCM I-4034, Bifidobacterium breve CNCM I-4035 and Lactobacillus rhamnosus CNCM I-4036. These strains were shown to adhere to intestinal mucus in vitro, to be sensitive to antibiotics and to resist biliary salts and low pH. In the present study, a multicenter, randomized, double-blind, placebo-controlled trial with 100 healthy volunteers in three Spanish cities was carried out to evaluate the tolerance, safety, gut colonization and immunomodulatory effects of these three probiotics. Volunteers underwent a 15-day washout perio…
The “central dot sign” in acute epiploic appendagitis
2018
Epiploic appendagitis is the acute inflammation and infarction of epiploic appendages, commonly secondary to torsion. It is a great mimicker as its clinical manifestation may be similar to appendicitis, diverticulitis, or colitis. Focal abdominal pain, tenderness, and mild pyrexia are clinical features of the condition [1]. Epiploic appendages are pedunculated adipose structures which extend from the serosal surface of the colon, along the free taenia and taenia omentalis. They are abundant in the caecum and sigmoid colon and absent in the rectum
Intestinal tuberculosis in a child living in a country with a low incidence of tuberculosis : a case report
2014
Background: Relatively common in adults, intestinal tuberculosis is considered rare in children and adolescents. The protean manifestations of intestinal tuberculosis mean that the diagnosis is often delayed (sometimes even for years), thus leading to increased mortality and unnecessary surgery. The main diagnostic dilemma is to differentiate intestinal tuberculosis and Crohn’s disease because a misdiagnosis can have dramatic consequences. Case presentation: A 13-year-old Caucasian, Italian female adolescent attended the Emergency Department complaining of abdominal pain, a fever of up to 38°C, night sweats, diarrhea with blood in stool, and a weight loss of about three kilograms over the p…
