Infliximab three-dose induction regimen in severe corticosteroid-refractory ulcerative colitis: Early and late outcome and predictors of colectomy
2014
Abstract Background Infliximab is effective as rescue therapy in severe corticosteroid-refractory ulcerative colitis. The optimal dose regimen and the long term benefits are not well defined. The aim of the present study was to evaluate short- and long-term colectomy rate in a cohort of patients with severe corticosteroid-refractory ulcerative colitis who received a three-dose infliximab induction regimen. Methods One hundred and thirteen patients admitted to 11 Italian IBD referral centres and treated with infliximab according to an intention to treat three-dose regimen were included. The co-primary endpoints were 3- and 12-month colectomy rate. The secondary end-points were the overall co…
Refinements in buccal mucosal graft urethroplasty for hypospadias repair
1998
We evaluated our experience with buccal mucosa in urethral reconstruction for complex hypospadias repair in a total of 62 children operated on at our institution since 1990 at a mean follow-up of 23 months. After the graft had been taken from the lower lip, with possible extension to the inner cheek, complete thinning of the graft was performed. The graft was then sutured in an onlay fashion to the urethral plate, and suprapubic urinary diversion was carried out for 3 weeks. The overall complication rate in this rather complex series of patients, including 26 "hypospadias cripples," was 13%; the complications involved 4 fistulas, 3 cases of graft necrosis, and only 1 meatal stenosis during …
Electromagnetically Generated Extracorporeal Shock Wave Lithotripsy and Adjuvant Combined Oral Litholysis for Therapy of Symptomatic Gallbladder Ston…
1991
A prospective study was conducted to evaluate effectivity, problems and adverse effects of extracorporeal shock wave lithotripsy (ESWL) using a newly developed electromagnetic biliary lithotriptor (Lithostar Plus, Siemens, Erlangen, FRG) for the treatment of selected patients presenting with symptomatic cholecystolithiasis. In addition to generally accepted criteria for the selection of patients, gallbladder contractility was established and pigment stones were excluded by computed tomography (CT). 80 out of 486 patients (63 females, 17 males, mean age 36, range 17-76 years) were selected for ESWL using a standardized diagnostic program. 62 out of 80 patients participating in the study had …
Treatment of human brucellosis with rifampin plus minocycline
2003
In order to evaluate the efficacy and tolerability of a high intravenous dose of rifampin plus oral minocycline (administered daily for 3 weeks) for the treatment of acute brucellosis, we retrospectively reviewed the outcome of 239 consecutive patients (135 adults and 104 children) diagnosed and treated over a 17-year period in Italy. The combination used resulted in 100% response and a relapse rate lower than 2%. Fifty-two (30 adults and 22 children) (29.8%) complained of mild adverse effects including an increase in aspartate aminotransferase (>250 IU) observed in 12 cases and considered related to rifampin and in 11 cases a reversible hyperpigmentation of the tongue attributed to minocyc…
Stroop task performance across the lifespan: High cognitive reserve in older age is associated with enhanced proactive and reactive interference cont…
2020
Abstract Susceptibility to interference increases with age but there is large inter-individual variability in interference control in older adults due to a number of biological and environmental factors. The present study aims at analyzing behavior and ERPs in a Stroop interference task with increasing difficulty in a sample of 246 young, middle-aged and healthy old participants. The old age group was divided into three subgroups based on performance scores. The results show a gradual performance reduction with increasing age and task difficulty. However, old high performers reached a performance level comparable to middle-aged subjects. The contingent negative variation (CNV) reflecting pr…
Ventilatory threshold during incremental running can be estimated using EMG shorts
2012
The present study examined whether shorts with textile electromyographic (EMG) electrodes can be used to detect second ventilatory threshold (V(T2)) during incremental treadmill running. Thirteen recreationally active (REC) and eight endurance athletes were measured for EMG, heart rate, blood lactate and respiratory gases during VO(2max) test (3 min ramps, 1 km·h(-1) increments). V(T)(2), onset of blood lactate accumulation (OBLA) and EMG threshold (EMG(T)) were determined. In athletes, OBLA occurred at 56 ± 6 mL·kg(-1)·min(-1), V(T2) occurred at 59 ± 6 mL·kg(-1)·min(-1), and EMG(T) at 62 ± 6 mL·kg(-1)·min(-1) without significant differences between methods (analysis of variance: ANOVA). In…
Neuromuscular characteristics and fatigue in endurance and sprint athletes during a new anaerobic power test
1994
The purpose of this study was to investigate neuromuscular and energy performance characteristics of anaerobic power and capacity and the development of fatigue. Ten endurance and ten sprint athletes performed a new maximal anaerobic running power test (MARP), which consisted ofn x 20-s runs on a treadmill with 100-s recovery between the runs. Blood lactate concentration [la−]b was measured after each run to determine submaximal and maximal indices of anaerobic power (P 3mmol·1 −1,P5mmol·1 −1,P10mmol·1 −1andP max) which was expressed as the oxygen demand of the runs according to the American College of Sports Medicine equation: the oxygen uptake (ml·kg−1·min−1)=0.2·velocity (m·min−1) +0.9·s…
No change in apolipoprotein AI metabolism when subcutaneous insulin infusion is replaced by intraperitoneal insulin infusion in type 1 diabetic patie…
2006
In type 1 diabetic patients, the replacement of subcutaneous insulin infusion by intraperitoneal insulin infusion restores the normal physiological gradient between the portal vein and the peripheral circulation, which is likely to modify HDL metabolism. This stable isotope kinetic study was designed to compare HDL apolipoprotein (apo) AI metabolism in seven type 1 diabetic patients first treated by continuous subcutaneous insulin infusion by an external pump and then 3 months after the beginning of intraperitoneal insulin infusion by an implantable pump. Glycaemic control was comparable under subcutaneous and intraperitoneal insulin infusion (HbA1c=7.34+/-0.94% versus 7.24+/-1.00%, NS). HD…
Apomorphine-Induced Growth Hormone Response Is Attenuated by Ethanol but Not Dextromethorphan
2007
Background: Misuse of alcohol drinking is a major health problem. Alcohol decreases spontaneous growth hormone (GH) secretion, but the mechanism is unclear. The aim of this study was to test whether administration of alcohol (study 1) or a N-methyl d-aspartate (NMDA) receptor antagonist (study 2) attenuates the GH response to pharmacological dopaminergic stimulation. Methods: The 2-session repeated measures design was conducted at the endocrine laboratory at the Department of Psychiatry at the Free University Berlin. Twenty healthy Caucasian males aged 35±10 years without a history of alcohol use disorders were tested using the Apomorphine (APO) challenge test. In study 1, we injected APO (…
Critical issues for employees in inter-municipal health care services: a multiple case study
2018
Abstract Background Traditional, hierarchical government structures have recently been challenged by increased complexity, fragmented services and heavy public demand. When healthcare services become fragmented and decentralised, they require redesign. Inter-municipal cooperation is a strategy to deal with current challenges and future demographic changes. Few studies exist that can help us conceptualize challenges regarding employment in this context and inform managers in the involved municipalities. This study aims to identify critical issues for employees in inter-municipal health care services and to elaborate on how and why these issues are experienced. Methods A multiple qualitative …
