On the Randomisation Gallery:Random Reflections on a Popular Method in Medical Research

2016

Objective: Randomized controlled trials (RCTs) and meta-analysis are increasingly utilized methods in medicine. There is, however, substantial critique on both from the theoretical as well as from the practical side. Methods: Seven points on RCTs and three on meta-analysis were choosen to summarize the current critique. Studies critically reviewing the quality of meta-analyses in various fields of medicine were narratively reviewed. Results: The typical RCT is short, performed on a small group of selected patients without severe comorbidity. It is expensive and favors drugs over other forms of therapies. Furthermore, neither patients nor doctors much like RCTs, they feel restricted in their…

Diagnosis, treatment, and analysis of long-term outcomes in idiopathic normal-pressure hydrocephalus

2007

To the Editor:We read with interest the article by McGirt et al. (9) entitled“Diagnosis, Treatment, and Analysis of Long-term Outcomes inIdiopathic Normal Pressure Hydrocephalus.” The authors con-clude that gait impairment is the primary symptom that inde-pendently predicted improvement after shunting. The studyincluded 132 patients, 129 (98%) of whom had gait impairmentas a feature. We would like to comment on this particular selec-tion criterion, as well as improvements in the Mini Mental StateExamination (MMSE) as the sole measure of postoperative cog-nitive improvement. One of the inclusion criteria for this study was presentationwith two or more features of the classic triad. Thus, the …

Delirium Assessment in Older People in Emergency Departments. A Literature Review.

2018

Delirium is a neuropsychiatric syndrome often manifesting in acute disease conditions, and with a greater prevalence in the older generation. Delirium in the Emergency Department (ED) is a highly prevalent problem that typically goes unnoticed by healthcare providers. The onset of a delirium episode in the ED is associated with an increase in morbidity and mortality. Because delirium is a preventable syndrome, these statistics are unacceptable. Emergency Department staff therefore should strive to perform systematic screening in order to detect delirium. Different tools have been developed for the assessment of delirium by healthcare professionals other than psychiatrists or geriatricians. …

Ageing and sexuality

2016

Abstract Sexuality is a dimension that concerns human health with profound implications not only in the biological and psychological aspects, but also in the social and cultural dimensions, affecting all ages of life. Sexuality in old age is still conditioned by biases, prejudices and from a stereotyped vision, which considered older people as “asexual”, in spite of several studies and surveys showing that older persons have sexual potential to express. In population surveys, a fair number of men and women aged over 60 years reported having sex at least once a month. The most influential predictor of sexual activity seems to be the physical health in older men, and the quality of the relati…

Use of statins and the risk of dementia and mild cognitive impairment: a systematic review and meta-analysis

2018

AbstractWe conducted a systematic review and meta-analysis to investigate whether the use of statins could be associated with the risk of all-caused dementia, Alzheimer’s disease (AD), vascular dementia (VaD), and mild cognitive impairment (MCI). Major electronic databases were searched until December 27th, 2017 for studies investigating use of statins and incident cognitive decline in adults. Random-effects meta-analyses calculating relative risks (RRs) were conducted to synthesize effect sizes of individual studies. Twenty-five studies met eligibility criteria. Use of statins was significantly associated with a reduced risk of all-caused dementia (k = 16 studies, adjusted RR (aRR) = 0.849…

Cognitive screening tests as experienced by older hospitalised patients: a qualitative study

2011

Older people admitted to geriatric wards in hospitals are often screened for cognitive impairments. The validity and diagnostic concerns of cognitive screening tests have been subjected to comprehensive research. However, the qualitative knowledge available on how older patients themselves experience these screening tests is limited. The aim of this study is to explore the cognitive screening test experience from the older patients' perspective. Drawing on fieldwork, qualitative interviews were performed with 18 older patients who had completed cognitive screening tests while hospitalised. Data from the interviews were analysed according to a phenomenological approach. The results were supp…

Mild cognitive impairment: classification method and procedure

2012

Title: Mild cognitive impairment: classification method and procedure. Abstract: Mild cognitive impairment (MCI) is one of the most important clinical entities in the field of cognitive neuroscience, the size of their numbers and the possible evolution of some of the subtypes of dementia. The aim of this study is to test a method and procedure for classification of MCI in terms of neuropsychological test scores and its convergence with established clinical criteria, and analyze the differences between the resulting groups. After applying the inclusion-exclusion criteria, was as- sessed using a battery of neuropsychological tests to 39 people aged be- tween 60 and 89 years. Diagnostic groups…

Detecting impaired language processing in MCI patients using around-the-ear cEEgrid electrodes

2021

AbstractMild cognitive impairment (MCI) is the term used to identify those individuals with subjective and objective cognitive decline but with preserved activities of daily living and an absence of dementia. While MCI can impact functioning in different cognitive domains, most notably episodic memory, relatively little is known about the comprehension of language in MCI. In this study we used around-the-ear electrodes (cEEGrids) to identify impairments during language comprehension in MCI patients. In a group of 23 MCI patients and 23 age-matched controls, language comprehension was tested in a two-word phrase paradigm. We examined the oscillatory changes following word onset as a function…

Effect of probiotics as a complement to non-surgical periodontal therapy in chronic periodontitis : a systematic review

2020

Background To improve the results of the classic periodontal treatment, probiotics have been suggested recently to decrease the number of bacteria and the expression of mediators of inflammation. This systematic review aimed to assess the literature for the effectiveness of different probiotic strains as adjuvants to non-surgical periodontal therapy. Material and Methods The electronic database of MEDLINE (via Pubmed) was searched up to December 2017 for randomised clinical trials in English comparing non-surgical periodontal treatment and probiotics versus periodontal treatment and placebo. The primary outcome investigated was reduction in pocket probing depth. Secondary outcomes were blee…

The use of pedicled buccal fat pad combined with sequestrectomy in bisphosphonate-related osteonecrosis of the maxilla

2010

The use of pedicled buccal fat pad flap (BFP) has proved of value for the closure of oroantral and oronasal communications and is a well-established tool in oral and maxillofacial surgery. Otherwise, the perceived limitations of surgical therapy for bisphosphonate-related osteonecrosis of the jaws (BRONJ) have been widely discussed, and recommendations have largely been made to offer aggressive surgery only to stage 3 patients refractary to conservative management. Oroantral communication may be a common complication after sequestrectomy and bone debridement in upper maxillary BRONJ. We report a case series of stage 3 recalcitrant maxillary BRONJ surgically treated with extensive sequestrec…
