Nursing home vouchers in Spain: the Valencian experience
2002
The main purpose of this paper is to evaluate the innovative nursing home voucher scheme that was introduced by the Valencia Autonomous Region of Spain in the early 1990s to implement targets laid down by the national plan for the development of older people's services. The article begins with a review of the evolution of Spanish social services, and especially the nursing home sector, and then summarises the genesis, objectives and characteristics of the voucher scheme. The main part of the paper reports a performance analysis and economic evaluation of the programme. On the basis of detailed information over four years, it is concluded that nursing home vouchers have contributed to the in…
Energy Saving Optimizing the Ventilation Control in a Big Shopping Center
2018
The paper investigates the potential energy savings in the context of big shopping centers, thanks to the introduction of innovative technologies able to control the ventilation according the real requirement for comfort and health of indoor spaces. The case study analyses a big shopping center, where the ventilation is realized with a centralized air-conditioning plant, working with fixed air flows. Two solutions are proposed to reduce the energy consumption for ventilation. The first considers the installation of heat recovery exchangers, the second the introduction of a control system able to modulate the air exchange, according to the real requirements. An energetic and economic evaluat…
Assessment of the renewable energy production in a historical building for a touristic intended use: a technical-economic feasibility study
2011
This column deals about a study based on technical and economical feasibility of a combination of technologies and systems for energy production concerning electric, heating and cooling to install on a building in the historic centre of a Sicilian city (Italy). After a functional review and an energetic evaluation, the building will have a hotel destination. A first evaluation, made on a model set with the geometrical, thermo-physical and climatic data both of the building and the place, allowed to make a dynamic simulation to define the energetic monthly needs. The evaluation has been done considering different solutions and technologies. With the collected data we supposed a first sizing …
A new decision model for economic evaluation of novel therapies for HCV
2014
In 2014, the European Medicines Agency (EMA) has given the license to two new direct-acting antiviral: sofosbuvir and simeprevir. The evidence provided by the studies, reported a high rate of SVR even in patients with decompensated cirrhosis. This and other innovative elements are potentially adept at changing the entire natural course of HCV. However, the dramatic prevalence rates of HCV observed in Italy, and the high prices that are expected to be required by the pharmaceutical industry, raises some critical issues about how to regulate access to such drugs. The objective of this article is to present a new decision model for the evaluation of novel therapies for HCV. This model is inten…
Telemedicine: Economic and clinical evaluation of the management of ischemic stroke
2017
Summary Introduction Ischemic stroke (IS) is a therapeutic emergency that requires prompt care in a stroke unit (SU). In order to reduce regional inequalities and to promote uniformity in access to this technique, which is effective when carried out quickly, telemedicine-assisted thrombolysis was developed in Burgundy within the Telestroke system. At the moment, no study compared costs (i.e. expenditures) and revenues (i.e. funding) associated with the management of patients presenting an IS in Burgundy after the implementation of the Telestroke system according to the management strategy: patients undergoing thrombolysis directly in a SU or Tele-thrombolysis in a local hospital (LH) receiv…
Profiling Major Sport Event Visitors: The 2002 Commonwealth Games
2007
It has become common practice for governments and municipalities around the world to bid for the right to host a major sporting event. Prior to embarking on the bidding process, politicians attempt to determine whether such an event will be of value to their municipality; and often focus on the estimated economic consequences of hosting such an event. Frequently, studies are commissioned to predict the event's economic value. However, these studies often miscalculate the potential impact of sport event visitors as consumers. We argue that enhanced profiling of these visitors will enable a more accurate assessment of economic impact. The current research surveys 1,196 spectators of the 2002 …
Theories and methods to assess the social cost of occupational risk factors in France : application to the case of occupational cancers
2011
The main objective of this thesis is to evaluate the social cost of respiratory cancer attributable to occupational risk factors in France. A summary of the available epidemiological data in the literature is performed. By using the Medline database, a review of the literature restricted to meta-analysis highlights the relative risk data available. The method of Attributable Risks (AR) is mobilized to estimate the numbers of lung, sinonasal and mesothelioma cancer cases caused by asbestos, exhaust fumes from diesel engines, painters, crystalline silica, wood dust and leather dust. We then assess the costs of these cancer cases for the french society using the Cost Of Illness (COI) method. T…
Enhancing values of roofs albedo for lowering cities’ air temperature and electric demand of buildings: a simple economic evaluation
2019
Abstract In cities vegetated roofs are becoming more popular because they can mitigate Urban Heat Island phenomena by decreasing the outdoor air temperature in summer. This decrease reduces the electric energy demand for climatization of buildings, which, in front of a milder climate, will recur less to mechanical tools for guaranteeing thermal comfort conditions to occupants. Cities can registered another indirect positive effect: the reduced cooling energy demand, limits the heat released by the climatization systems’ external unities toward the urban open spaces, thus lowering the outdoor air temperature. Therefore, the outdoor surface temperature of green, as well as cool roofs, can be …
Economic evaluation of caspofungin vs liposomal amphotericin B for empirical therapy of suspected systemic fungal infection in the German hospital se…
2007
As antifungal agents are frequently used in hematology and oncology, economic data on the empirical therapy of suspected systemic fungal infection are pivotal. Data were analyzed according to: (1) the rate of nephrotoxicity related to treatment with caspofungin in comparison to liposomal amphotericin B (L-AmB) from a randomized clinical trial, (2) the effect of nephrotoxicity on length of hospital stay from a European observational study, and (3) an example of total bottom-up cost in a department of hematology in Germany. All estimates include 95% confidence intervals (CI) using two-stage Monte Carlo simulation on binominal and Gaussian random variables from separate studies with comparable…
