For better comprehension of the metabolic syndrome, it is necessary to differentiate the effect of insulin on glucose metabolism on the one hand, and on other metabolic activities on the other hand. Whereas glucose utilization is affected by insulin resistance, the effect of insulin on lipid metabolism, ion and aminoacid transport does not seem to be diminished. Lipid metabolism, however, seems to play a crucial role in the induction of the vicious cycle. Important targets for cAMP signalling in the heart are hyperpolarization-activated and cyclic nucleotide-gated (HCN) channels that underlie the depolarizing 'pacemaker' current, I(f). We studied the role of I(f) in mice, in which binding of cAMP to HCN4 channels was abolished by a single amino-acid exchange (R669Q). Homozygous HCN4(R669Q/R669Q) mice die during embryonic development. Theoretical approach and clinical application of kinetic modelling in dialysis.

Using kinetic modelling for shaping profile dialysis, we present a promising approach to improve the cardiovascular stability of patients during dialysis treatment. In order to obtain an insight into the physiological mechanisms of increased stability, a model considering alterations of electrolytes and water distribution and of acid-base status was developed. This algorithm was used for the evaluation of 114 dialysis sessions, which were performed with highly individualised profiles. Dilation of the ductus venosus by stent implantation increases placental blood perfusion in fetal sheep.

The reduction of resistance to flow in the ductus venous (DV) and a decrease of blood supply to the liver serve for the survival of the fetus during hypoxia. The present study investigated the influence of the increased diameter of the DV on placental blood perfusion.In 15 ewes with twin pregnancies at gestational ages of 117 +/- 4 days, a stent (4 or 5 mm) was placed into the DV of 1 twin (DV(stent) group) under ultrasound guidance. Corneal opacity and copper levels of the Lewis syndrome after systemic chemotherapy

Abstract Purpose To report a female patient of biclonal Lewis syndrome which consists of a trias: biclonal gammopathy of undetermined significance, paraproteinemic keratopathy in form of a brownish discoid opacification at the level of Descemet's membrane and hypercupremia. After several years there was a conversion to multiple myeloma.
