Sir: The prognosis of stage I and -II Wilms tumours with favourable histology is good, reaching 97% (stage I) and 92% (stage II) 5 year disease-free survival rates [1]. With these figures in mind the question arises whether the overall prognosis of Wilms tumours could be improved by presymptomatic routine physical examinations in an attempt to avoid stage III to V primary diagnoses. In the Federal Republic of Germany six routine physical examinations (RPE) are offered to infants, and two additional ones to children between 12 and 48 months. Improvement of trospium-specific absorption models for fasted and fed states in humans

The purpose of this study was to mechanistically interpret the oral absorption pattern of trospium in fasted and fed states by means of gastrointestinal simulation technology. A drug absorption model was built on the basis of experimental data. According to the generated model, low permeability across the intestinal epithelium, delayed gastric emptying time and a prolonged residence time in the small intestine are the key factors governing trospium absorption in the fasted state. Furthermore, in silico modelling provided a plausible explanation of the pronounced reduction in the oral bioavailability of trospium when administered with food. The article relates Kant to the American Revolution by connecting his republicanism with the ingenious balancing of national and federal features in the constitution of the American republic, as envisioned and enacted by James Madison. Section 1 sets the stage by profiling Kant and Madison in historical and systematic terms. Section 2 traces the development of republicanism from its ancient origins to its modern variety. Section 3 reconstructs Kant's legalist republicanism in the twofold context of his philosophy of history and his philosophy of law. Section 4 portrays Madison as the leading theoretician of the modern federal republic.
