High-dose immunoglobulins (IVIG) are important agents in the treatment of numerous autoimmune disorders. In the field of dermatology, increasing numbers of patients with bullous autoimmune disorders and mixed connective tissue diseases have been treated. We successfully treated a patient with pemphigus vulgaris with IVIG, after he had developed osteonecrosis of his right femoral head while on azathioprine and corticosteroids. After reduction of oral corticosteroids, his disease flared dramatically. The addition of a high dose immunosuppressive regimen including azathioprine and dapsone failed to help. AIM Decreases in depth and irregularity are typical changes of the acetabulum in patients with Perthes disease and develop secondary to the femoral head involvement. Optimal timing of therapy plays an important role to prevent these secondary changes. AIM OF THE STUDY The results from 56 patients with osteonecrosis of the femoral head who underwent 66 uncemented total hip replacements were analyzed and compared with current reports in the literature on cemented endoprosthesis. METHODS In the retrospective study in 30 of those cases the Zweymuller and in 25 cases the HA-coated ABG prosthesis were used. In addition the results of 11 patients with a polyethylene Endler cup in combination with a Zweymuller stem are presented. The mean age at surgery was 48 years, the mean follow-up was 5.9 years. Acetabular fractures in elderly patients are rare injuries, but their incidence is increasing. Poor bone quality due to osteoporosis and an increased operative risk due to concomitant disease are factors complicating surgical therapy. Literature does not provide generally accepted treatment protocols. In a 4-year period, 27 patients who were 65 years or older and who had an acute displaced fracture of the acetabulum were admitted to our department. Four minimally displaced and stable fractures were managed conservatively. Internal fixation was performed in 16 cases. Total hip replacement is often recommended for the treatment of femoral neck fractures in elderly patients. A less invasive approach with shorter operating time is the replacement of the femoral head only, leaving the acetabular cartilage in situ. Hemiarthroplastic hip replacement using a metallic head prosthesis was not successful in the past, because the majority of patients developed protrusio acetabuli after surgery. Zur Diagnostik der transitorischen Osteoporose der Hüfte und deren Darstellbarkeit in der MR-Tomographie

Magnetic resonance (MR) imaging may distinguish transient osteoporosis of the hip from more severe hip diseases. 12 patients with transient osteoporosis of the hip underwent MR imaging. The initial and follow-up studies included T1- and T2-weighted images in coronal, transverse or sagittal planes. There were revealed 3 stages: diffuse-, focal-, and residual stage. This syndrome is characterized by hip pain, limping, and osteoporosis of the femoral head with preservation of the joint space. The clinical symptoms and abnormalities on MR images disappeared completely within 6-10 months.
