Alternative surgical management of oroantral fistula using auricular cartilage

One of the clinical complications encountered by oral and maxillofacial surgeons is oroantral communication (OAC) with subsequent formation of oroantral fistula (OAF). Many techniques and treatment modalities have been described for the management of OAC and OAF. There are advantages and disadvantages of all these techniques. We report a 21-year-old male patient who was admitted to our department for the presence of an OAF and was treated using an auricular cartilage graft. This technique may be useful to treat OAF and to provide a solid alveolar bone site for subsequent pre-implant surgery. Key words:Auricular cartilage, implant surgery, oroantral fistula.

Background Removing a tooth from the jaw results in the occurrence of oroantral communication in beneficial anatomic conditions or in the case of a iatrogenic effect. Popularized treatments of the oroantral communication have numerous faults. Large bone defect eliminates the chance to introduce an implant. A modified prosthesis for the treatment of malignant esophagotracheal fistula

Esophagotracheal fistula is usually a sequela of irradiation or laser treatment of advanced carcinoma of the esophagus or the tracheobronchial tree. Resection of the tumor in these cases is not possible, and palliative bypass surgery is highly risky. The peroral placement of a prosthesis is less invasive, but conventional prostheses often fail to occlude the fistula. The authors regularly use an endoscopic multiple-diameter bougie for dilation. After dilation, a specially designed prosthesis is pushed through the tumor stenosis to block the fistula. This procedure can be done without general anesthesia. In the past few years, the use of flow diverter devices was extended to several other “off-label” indications, including solitary or adjunctive treatment to coil embolization for direct (Barrow A type) carotid cavernous fistulas (CCFs). Cerebrospinal fluid fistulas in the otologic pathology show the presence of one or more anomalous touches between subaracnoids spaces and temporal bone. Cerebrospinal fluid fistulas are distinguished in congenital and acquired. The acquired fistulas are the most large group and they include post-traumatic fistulas, post-infective fistulas and/or post-inflammatory fistulas, iatrogenic fistulas, post-neoplastic fistulas and spontaneous fistulas. The congenital fistulas are more rare, they involve otic capsula and are, often, associated with malformations of inner ear.
