Kinematics of the patellofemoral joint. Investigations on a computer model with reference to patellar fractures.

1990

Patellofemoral kinematics were studied on a computer model. The articulating point of the patella moves from distal to proximal during flexion, until the quadriceps tendon starts to turn around the femur tendo-femoral gliding. The pattern of patellofemoral movement consists of a gliding and rolling component, the latter at its maximum at the beginning of flexion. The pathway of the instant centers of motion moves from dorsal to ventral during flexion. Stress analysis shows that the contact area is at all times the area of maximum bending stress. The distal part of the patella is the part with mechanical stress at all angles of flexion. The conclusions to be drawn for treatment are that frac…

Reconstruction of vertebral body in thoracolumbar AO type A post-traumatic fractures by balloon kyphoplasty: a series of 85 patients with a long foll…

2019

Background Traumatic fractures of the thoracolumbar spine are common injuries, accounting for approximately 90% of all spinal traumas. Optimal management of these fractures still gives rises to much debate in the literature. Currently, one of the treatment options in young patients with stable traumatic vertebral fractures is conservative treatment using braces. Kyphoplasty as a minimally invasive procedure has been shown to be effective in stabilizing vertebral body fractures, resulting in immediate pain relief and improved physical function with early return to work activity. The aim of the study is to report VAS, ODI scores, and kyphosis correction following treatment. Methods This is a …

Tracer migration in crystalline rock : application to geological barriers of nuclear waste storage

2016

This work deals with transport of radionuclides in the geosphere. The subject is investigated through characterisation of water conducting fractures, determining of rock transport properties and development of new methods for characterising geological samples. Here, as is often the case, radionuclide transport is investigated indirectly by characterising the structures, where the transport takes place, and directly by measuring transport properties in the gas phase. First water conducting fractures of three different types and the rock matrix surrounding them were analysed through rock samples from Olkiluoto. The analysis was done using X-ray tomography, the 14C-PMMA autoradiography techniq…

Microsurgery and external fixation in orthoplastic reconstruction of tibial injuries.

2019

"Orthoplastics" is a relatively new approach to lower limb reconstruction, where an integration of both plastic and orthopedic expertise is required, together with the availability of well-equipped facilities. Acute shortening and long-term frames for lengthening are generally considered alternatives to length preservation and soft tissue microsurgical reconstruction, but an integration of external fixation and reconstructive microsurgery is gaining an increasing role with refinements of joint approaches.Data on sixteen patients who underwent microsurgical lower limb reconstruction and external fixation with an orthoplastic approach, following acute or chronic tibial injury, were retrospect…

Dynamics of hydrofracturing and permeability evolution in layered reservoirs

2015

International audience; A coupled hydro-mechanical model is presented to model fluid driven fracturing in layered porous rocks. In the model the solid elastic continuum is described by a discrete element approach coupled with a fluid continuum grid that is used to solve Darcy based pressure diffusion. The model assumes poro-elasto-plastic effects and yields real time dynamic aspects of the fracturing and effective stress evolution under the influence of excess fluid pressure gradients. We show that the formation and propagation of hydrofractures are sensitive to mechanical and tectonic conditions of the system. In cases where elevated fluid pressure is the sole driving agent in a stable tec…

Effects of home-based physical exercise on days at home, healthcare utilization and functional independence among patients with hip fractures : a ran…

2021

Objective To evaluate the effects of a physical exercise program on days lived at home, the use and costs of healthcare and social services mortality and functional independence among patients with hip fractures. Design Randomized controlled trial with parallel two-group design, consisting of a 12-month intervention and 12-month registry follow-up. Setting Home-based intervention. Participants Patients with operated hip fracture, living at home, aged ≥60 years, randomized into physical-exercise (n=61) or usual-care (n=60) groups. Intervention Supervised physical exercise twice a week. Main Outcome Measures The primary outcome was the number of days lived at home over 24 months. Secondary ou…

The importance of vitamin D3 supplementation in orthopedics

2020

Falls have been recognized by the WHO as one of the most significant problems in aging societies. In the elderly after 65 years of age, they cause 90% of the fractures of the proximal femoral bone (bkk) and 100% of the forearm fractures [5].They usually occur when walking (most often), getting up and sitting down. The consequences of falls, as well as themselves, can be prevented by supplementing vitamin D3 [6] [8] regulating the body's calciumphosphate homeostasis. It improves neuromuscular function, including the mechanism reduces the frequency of falls. It also affects response time and balance. The concentration of vitamin D3 in the body is variable. Depends on endogenous biosynthesis a…
