Appropriate management of growth hormone deficiency during the age of transition: an Italian Delphi consensus statement

2022

Growth hormone deficiency (GHD) describes the impairment of growth hormone (GH) secretion by the pituitary somatotroph cells. GHD may be congenital, with causes, including genetic alterations and structural brain malformations, or acquired, including midline tumours, cranial irradiation, traumatic brain injury, central nervous system infections and inflammatory conditions. GHD in children is characterised by short stature, delayed bone maturation and abnormalities in substrate metabolism, body composition, physical and psychosocial functioning, all of which improve with recombinant human GH (rhGH) therapy. The diagnosis of GHD is based on clinical signs and symptoms, biochemistry and imagin…

Revaluation of the clinical and metabolic behavior of GHD children during GH treatment according to newly proposed note 39 of the Italian Medicines A…

2015

Background The newly proposed note 39 of the Italian Medicines Agency (AIFA) revisited the diagnostic criteria of growth hormone deficiency (GHD) in children and, applying it, a percentage of children previously diagnosed as GHD may have received a wrong diagnosis and a unnecessary treatment, with potential clinical implications. Aim To evaluate the clinical and metabolic behavior of GH-treated children according to the new criteria of GHD diagnosis. Subjects and Methods: We retrospectively analyzed clinical and metabolic data of 310 prepubertal children (220 M, 90 F, age 10.8 ± 2.9 yrs) with short stature admitted to our section of Endocrinology during the years 2005-2014, having at least …

Clinical and metabolic impact of three-times-weekly versus daily growth hormone (GH) treatment in naïve GH-deficient children

2017

OBJECTIVE: Growth hormone treatment (GHT) is commonly administered daily, although pulsatile GH secretion is unlikely to be achieved. The auxological effect of a three-injections-per-week (TIW) regimen is controversial, while the metabolic effects have been never evaluated in children. The objective was to evaluate whether two different regimens of weekly injections could lead to similar auxological and metabolic effects in children with GH deficiency (GHD). DESIGN: 32 GHD children (25 males, mean age 10.5 ± 2.2 yr) were randomly assigned to receive daily (group A, No 16) or TIW (group B, No 16) GHT for 12 months. METHODS: Auxological parameters, insulin-like growth factor-I (IGF-I), glucos…

Disturbances in cholesterol, bile acid and glucose metabolism in peroxisomal 3-ketoacylCoA thiolase B deficient mice fed diets containing high or low…

2014

SPE IPM UB; International audience; : The peroxisomal 3-ketoacyl-CoA thiolase B (ThB) catalyzes the thiolytic cleavage of straight chain 3-ketoacyl-CoAs. Up to now, the ability of ThB to interfere with lipid metabolism was studied in mice fed a routinely laboratory chow enriched or not with the synthetic agonist Wy14,643, a pharmacological activator of the nuclear hormone receptor PPARα. The aim of the present study was therefore to determine whether ThB could play a role in obesity and lipid metabolism when mice are chronically fed a synthetic High Fat Diet (HFD) or a Low Fat Diet (LFD) as a control diet. To investigate this possibility, wild-type (WT) mice and mice deficient for Thb (Thb(…

The effect of nacom (L-dopa and L-carbidopa) on growth hormone secretion in 75 patients with short stature.

1977

The stimulatory effect of Nacom (250 mg L-Dopa and 25 mg L-Carbidopa) on the HGH secretion was evaluated in 75 short stature patients. The number of blood samples was restricted to only three (0, 45 and 90 min). 63 patients reached adequate HGH concentrations after the ingestion of 1 tablet Nacom (84%). Somatotropin levels increased from 2.08 (Sx 0.28) to a maximal HGH value of 14.22 (Sx 0.87) ng/ml. When the stimulatory effect of Nacom was compared with the standard method of arginine infusion in children with normal stature the arginine test was not superior to the Nacom-test. The Nacom-test appears to be a simple and reliable screening method for HGH deficiency, particularly in outpatien…

Oral administration of the growth hormone secretagogue NN703 in adult patients with growth hormone deficiency

2003

Summary objective Little is known of the usefulness of GH secretagogues (GHSs) in GH-deficient (GHD) adults. The objective of this study was to determine the number of responders to treatment with NN703 in GHD adults. design A multicentre, randomized, double-blind, and placebo-controlled study. patients Ninety-seven GHD adults were included. measurements The GH response before and after 1 week of oral treatment with NN703 (n = 83) or placebo (n = 14) was determined. The first and last dose of NN703 was 3 mg/kg, whereas the dose of NN703 was 1·5 mg/kg/day during the 6 days between the first and last doses. Furthermore, all 97 patients received 1 µg/kg GH-releasing hormone (GHRH) 3 weeks afte…

Glucose Metabolism in Children With Growth Hormone Deficiency

2018

Background: The growth hormone (GH)/insulin-like growth factor 1 (IGF-1) axis has a fundamental impact on glucose metabolism. Therefore, both untreated GH deficiency (GHD) and GH treatment (GHT) may be associated with some metabolic alterations, although the abnormalities of glucose metabolism have been investigated by relatively few studies as main outcomes. Aim: The present review summarizes the available data on glucose metabolism in children with GHD, providing an overview of the current state of the art in order better to clarify the real metabolic impact of GHD and GHT. Methods: Among all the existing studies, we evaluated all original studies that fulfilled our criteria for analysis …

Body mass reduction markedly improves muscle performance and body composition in obese females aged 61-75 years: comparison between the effects exert…

2004

OBJECTIVE: To investigate the effectiveness of a body mass reduction programme entailing diet caloric restriction and moderate physical activity with or without supplementary treatment with recombinant (r) GH or steroids to improve body composition and muscle performance in severely obese women aged 61-75 years. METHODS: Twenty women were randomly assigned to one of three groups: body mass reduction alone; body mass reduction plus rGH; body mass reduction plus nandrolone undecanoate. Body composition, isotonic muscle strength and anaerobic power output during jumping were determined before and after the 3-week period. RESULTS: Whatever the experimental group considered, body mass (P<0.01…
