Model-Based Decision Support System for Individual Prescription of the Dialysate Bicarbonate Concentration in Hemodialysis

A decision support system has been developed that determines the optimal dialysate bicarbonate concentration in hemodialysis therapy for each patient individually. The knowledge about the behavior of the acid-base state during treatment has been provided by a mathematical model for the description of dynamic exchange processes during hemodialysis. This model simulates the sodium and water distribution, the acid-base state as well as the ventilation. The decision support system uses the model for the prediction of the end-dialysis acid-base state and calculates by means of linear optimization the dialysate bicarbonate concentration which is necessary to reach a specified end-dialysis state.…

Digoxin and digitoxin elimination in man by charcoal hemoperfusion

Since there is no widely used causal means of reducing the severity of massive digitalis intoxication the capability of hemoperfusion with coated activated charcoal to remove toxicologically relevant amounts of digoxin and digitoxin was evaluated in vitro and in man. At a blood flow rate of 100 ml/min the digoxin clearance by hemoperfusion in vitro was 51±8 ml/min in comparison to 24.3±11.3 ml/min by hemodialysis. The average hemoperfusion clearance of digitoxin was 31.7±13.4 ml/min, whereas almost no digitoxin was removed by hemodialysis. Introduction: The majority of the end-stage renal disease patients begin hemodialysis with the central tunneled catheter as a permanent or bridge vascular access. The procedure of central tunneled catheter insertion can be complicated by exit site bleeding, prolonged tunnel healing, and infection. Molecular epidemiology and forensic genetics: application to a hepatitis C virus transmission event at a hemodialysis unit.

Molecular phylogenetic analyses are frequently used in epidemiologic testing, although only occasionally in forensics. Their acceptability is hampered by a lack of statistical confidence in the conclusions. However, maximum likelihood testing provides a sound statistical framework for the testing of phylogenetic hypotheses relevant for forensic analysis.
