Mortality and cardiovascular disease burden of uncontrolled diabetes in a registry-based cohort: the ESCARVAL-risk study
2018
Background: Despite the epidemiological evidence about the relationship between diabetes, mortality and cardiovascular disease, information about the population impact of uncontrolled diabetes is scarce. We aimed to estimate the attributable risk associated with HbA1c levels for all-cause mortality and cardiovascular hospitalization. Methods: Prospective study of subjects with diabetes mellitus using electronic health records from the universal public health system in the Valencian Community, Spain 2008–2012. We included 19,140 men and women aged 30 years or older with diabetes who underwent routine health examinations in primary care. Results: A total of 11,003 (57%) patients had uncontrolle…
The "Honeymoon Phase" in Children with Type 1 Diabetes Mellitus (T1DM): Frequency, Duration and Predictive Factors at Onset
2012
Abstract: Aim of thè study was: to analyze thè epidemiological features of paediatric T1DM at onset and their relation to remission frequency and duration in thè first year of disease, to assess clinical effìcacy of Glucose Evaluation Trial REMission (GETREM) protocol in terms of induction and maintenance of thè "honeymoon phase" and to evaluate Insulin Dose-Adjusted A1C values at onset [IDAA1C = HbAlc% + (Insulin U/Kg/die x 4)] as a predictor of remission. 181 patients less than 15 years of age were admitted at our Department for T1DM onset and were treated according to GETREM protocol in thè years 2008-2011. The following data were recorded at onset: age, sex, modality of onset according …
Patients' and physicians' preferences for type 2 diabetes mellitus treatments in Spain and Portugal: a discrete choice experiment.
2015
Objective To assess Spanish and Portuguese patients’ and physicians’ preferences regarding type 2 diabetes mellitus (T2DM) treatments and the monthly willingness to pay (WTP) to gain benefits or avoid side effects. Methods An observational, multicenter, exploratory study focused on routine clinical practice in Spain and Portugal. Physicians were recruited from multiple hospitals and outpatient clinics, while patients were recruited from eleven centers operating in the public health care system in different autonomous communities in Spain and Portugal. Preferences were measured via a discrete choice experiment by rating multiple T2DM medication attributes. Data were analyzed using the condit…
The Association between Cognitive Impairment and Diabetic Foot Care: Role of Neuropathy and Glycated Hemoglobin
2020
Diabetes mellitus is associated with impairment in cognitive functions which can complicate adherence to self-care behaviors. We evaluated the incidence of cognitive impairment in patients with diabetes mellitus to determine the strength of the association between diabetic foot (a complication that occurs in about 10% of diabetic patients), adherence to the clinician&rsquo
Vapaa-ajan liikunnan ja istumisen yhteys tyypin 2 diabeetikoiden sokeriaineenvaihduntaan
2018
Tyypin 2 diabeteksessa sokeriaineenvaihdunta on häiriintynyt, minkä seurauksena verensokeri nousee liian korkealle. Paastoglukoosi- ja sokerihemoglobiiniarvolla (HbA1c) voidaan kuvata diabeetikon hoitotasapainoa. Sairauteen vaikuttavat niin perintötekijät kuin elintavat. Säännöllisellä kuntoliikunnalla on todettu olevan myönteinen vaikutus tyypin 2 diabeetikoiden glukoosiarvoihin. Fyysisen passiivisuuden, kuten istumisen merkitystä sairauteen on vielä vähän tutkittu. Tämän tutkimuksen tarkoituksena oli selvittää, onko vapaa-ajan liikunnan ja istumisen määrällä yhteyttä tyypin 2 diabeetikoiden sokeriaineenvaihduntaan. Lisäksi tarkasteltiin diabeetikoiden vapaa-ajan liikuntaa ja istumismääriä…
Positive airway pressure (PAP) treatment reduces glycated hemoglobin (HbA1c) levels in obstructive sleep apnea patients with concomitant weight loss …
2021
Abstract: Patients with obstructive sleep apnea (OSA) are at increased risk of developing metabolic disease such as diabetes. The effects of positive airway pressure on glycemic control are contradictory. We therefore evaluated the change in glycated hemoglobin (HbA1c) in a large cohort of OSA patients after long-term treatment with positive airway pressure. HbA1c levels were assessed in a subsample of the European Sleep Apnea Database [n=1608] at baseline and at long-term follow up with positive airway pressure therapy (mean 378.9 +/- 423.0 days). In a regression analysis, treatment response was controlled for important confounders. Overall, HbA1c decreased from 5.98 +/- 1.01% to 5.93 +/- …

Continuous subcutaneous Insulin infusion leads to immediate, table, and long-term changes in metabolic control
2008
Background: Evaluations of continuous subcutaneous insulin infusion (CSII) usually focus on one pre- and one post-CSII measurement to assess metabolic therapy outcome. Aim: Extending this research, the aim of the present study was to provide a more fine-grained analysis of achieved glycaemic control. Methods: In 52 patients with type 1 diabetes (mean age of 37.85 years at CSII begin; s.d. ± 12.41), haemoglobin A1c (HbA1c) levels were assessed every 3 months over a period of 5 years (1 year before and 4 years after the introduction of CSII). Mixed models were utilized to describe changes in glycaemic control. Results: The pre–post course showed that already in the first quarter, a statis…
