Efficiency of Health Centers in Cambodia: Case Study in Takeo Province

2015

Financing and efficiency of primary health care are two major issues for the development and deepening of health universal coverage in a context of strong financial constraint. In Cambodia, the development of demandside financing schemes - Health Equity Fund and Community-Based Health Insurance ? provides an opportunity to enhance the health centers (HCs) activity and thus their performance, which are very low as underlined by the literature. Nevertheless, except our study, the literature on Cambodian HCs focuses on the analysis of their activity and financing without taking into consideration efficiency issues. From a field survey, our study is a contribution to fill this gap by estimating…

Incentive and Selection Effects of Medigap Insurance on Inpatient Care

2012

The Medicare program, which provides insurance coverage to the elderly in the United States, does not protect them fully against high out-of-pocket costs. For this reason private supplementary insurance, named Medigap, has been available to cover Medicare gaps. This paper studies how Medigap affects the utilization of inpatient care, separating the incentive and selection effects of supplementary insurance. For this purpose, we use two alternative estimation methods: a standard recursive bivariate probit and a discrete multivariate finite mixture model. We find that estimated incentive effects are modest and quite similar across models. On the other hand, there seems to be very significant …

The association between new onset atrial fibrillation and incident cancer—A nationwide cohort study

2018

A recent analysis showed an association with new onset atrial fibrillation (NOAF) and incident cancer among women. We aimed to examine the risk of cancer among patients with NOAF in general population. A retrospective cohort of 5130 patients with NOAF was identified from a random sample of one million subjects between 2005 and 2010 from Taiwan National Health Insurance Research Database. The standard incidence ratio of incident cancer and hazard ratios were calculated by modeling cumulative incidence with competing risk of death. During a mean follow-up duration of 3.4 years, 330 patients developed cancer. The standard incidence ratio of all malignancies was 1.41 (95% confidence interval 1.…

Single-Target Implicit Association Tests (ST-IAT) Predict Voting Behavior of Decided and Undecided Voters in Swiss Referendums

2016

Undecided voters represent a major challenge to political pollsters. Recently, political psychologists have proposed the use of implicit association tests (IAT) to measure implicit attitudes toward political parties and candidates and predict voting behavior of undecided voters. A number of studies have shown that both implicit and explicit (i.e., self-reported) attitudes contribute to the prediction of voting behavior. More importantly, recent research suggests that implicit attitudes may be more useful for predicting the vote of undecided voters in the case of specific political issues rather than elections. Due to its direct-democratic political system, Switzerland represents an ideal pl…

Users of home-care services in a Nordic welfare state under marketisation: the rich, the poor and the sick

2017

Stricter access to public services, outsourcing of municipal services and increasing allocation of public funding for the purchase of private services have resulted in a marketisation wave in Finland. In this context of a Nordic welfare state undergoing marketisation, this paper aims to examine the use of Finnish care services among older people and find out who are using these new kinds of private services. How wide is their use and do the users of private care services differ from those who are using public services? How usual is it to mix both public and private care services? The questionnaire survey data set used here was gathered in 2010 among the population aged 75 and over in the ci…

Risk of Stroke and Post-Stroke Adverse Events in Patients with Exacerbations of Chronic Obstructive Pulmonary Disease

2017

Background The risk and outcomes of stroke in patients with chronic obstructive pulmonary disease exacerbations (COPDe) remain unclear. We examined whether patients with COPDe faced increased risk of stroke or post-stroke outcomes. Methods Using Taiwan’s National Health Insurance Research Database, we identified 1918 adults with COPDe and selected comparison cohorts of 3836 adults with COPD no exacerbations and 7672 adults without COPD who were frequency matched by age and sex in 2000–2008 (Study 1). Stroke event was identified during 2000–2013 follow-up period. Adjusted hazard ratios (HRs) and 95% confidence intervals (CIs) of stroke associated with COPDe were calculated. In a nested cohor…

The financial burden of non-communicable chronic diseases in rural Nigeria: Wealth and gender heterogeneity in health care utilization and health exp…

2016

Objectives Better insights into health care utilization and out-of-pocket expenditures for non-communicable chronic diseases (NCCD) are needed to develop accessible health care and limit the increasing financial burden of NCCDs in Sub-Saharan Africa. Methods A household survey was conducted in rural Kwara State, Nigeria, among 5,761 individuals. Data were obtained using biomedical and socio-economic questionnaires. Health care utilization, NCCD-related health expenditures and distances to health care providers were compared by sex and by wealth quintile, and a Heckman regression model was used to estimate health expenditures taking selection bias in health care utilization into account. Res…

Epidemiology of urticaria in German children

2020

Background To date, robust epidemiological metrics as well as data on comorbidity in pediatric urticaria are lacking. They form the basis for the design of efficient healthcare. Methods Retrospective study to analyze epidemiological data in pediatric urticaria. The analysis is based on routine data of a health insurance company operating throughout Germany (DAK-Gesundheit). Insured people under 18 years of age who received at least one confirmed outpatient or inpatient urticaria diagnosis according to the ICD-10 classification in the years 2010 to 2015 were included in the analysis and compared to children without a corresponding diagnosis. Results Of 2.3 million insured individuals, 313,58…

Impact of an ambulatory physical activity program on balance and motor abilities of retirees: a prospective study

2020

International audience; BACKGROUND:Given the public health problem generated by posture and gait disorders, a preventive approach is essential.OBJECTIVE:We investigated the impact of an ambulatory physical activity program (APAP) on the motor skills of retirees.METHOD:Retirees ≥ 60 years were recruited in this prospective study. The APAP was supported by a French regional health insurance plan. Between inclusion (T0) and 12 weeks (T1), subjects participated in an APAP (1 hour/week) that included strengthening muscular exercises, balance work, joint flexibility exercises, one-leg-balance test (OLB), stimulation of the foot arch, and rise-from-the-floor (RFF). Timed-up-and-go test (TUG) durat…
