Outpatient treatments are actually the techniques of choice in the management of low-grade hemorrhoidal disease. Among these, rubber band ligation (RBL) and injection sclerotherapy (IS) are the most frequently performed. Both techniques are used, without one having been determined to be superior over the other. We analyzed the studies that compare these two techniques in terms of efficacy and safety in order to offer a proposal for treatment choice. RBL seems to be most efficient in terms of symptom resolution for second-degree hemorrhoidal disease and equal or superior for treatment of third-degree disease. During examination of the anal region and before doing the rectal examination, the hand is protected by a plastic glove. A number of disposable gloves of varying strengths are on the market. One may additionally use a finger cot on the forefinger, which is removed after the examination. The examination can also be done using a finger cot alone with the finger stuck through a piece of gauze that protects the rest of the hand. Right-handed examiners often elect to use the forefinger of the left hand for the rectal examination and keep the right hand uncontaminated for handling instruments.

One hundred patients with a history of hemorrhoidal disease and suffering from an acute hemorrhoidal attack were randomized into two parallel groups and treated with Daflon 500 mg* (D500) or placebo (PL) under double-blind conditions. Daflon 500 mg was administered at the dosage of three tablets bid the first four days and two tablets bid the following three days. Overall improvement of symptoms was greater in the D500 group than in the PL group, from D2 up to D7.
