Sequentielle transarterielle Chemoembolisation (TACE) des hepatozellulären Karzinoms (HCC) - Erfahrungsbericht eines einzelnen Zentrums über 5 Jahre

PURPOSE: To analyze the course of disease of patients treated with sequential TACE and to evaluate the dependent and independent prognostic factors for patient survival using the Cox Proportional Hazard Model. MATERIALS AND METHODS: 94 patients palliatively treated with TACE. Patients were selected if they had been treated at least 3 times. The TACE procedure was carried out at 8-week intervals using a suspension consisting of a fixed dosage of Mitomycin C (10 mg) and 10 ml Lipiodol. Follow-up investigations included contrast-enhanced multislice CT before and after TACE and assessment of the laboratory test results (i. e., blood count, liver enzymes, and coagulation). The goal of the Liver Imaging Reporting and Data System (LI-RADS) is to standardize the interpretation and reporting of liver observations on contrast-enhanced CT and MR imaging of patients at risk for hepatocellular carcinoma. Although LI-RADS represents a significant achievement in standardization of the diagnosis and management of cirrhotic patients, complexity and caveats to the algorithm may challenge correct application in clinical practice. SIUMB guidelines and recommendations for the correct use of ultrasound in the management of patients with focal liver disease

The present document describes the SIUMB (Italian Society of Ultrasound in Medicine and Biology) guidelines for the use of ultrasound in the management of focal liver disease. The aim of the paper is to provide a clinical practice guideline for Italian physicians who are approaching the ultrasound study of a focal liver lesion. Purpose To perform an international survey on current practices in imaging-based surveillance, diagnosis, staging, and assessment of treatment response for HCC. Background & Aims CYLD is a tumor suppressor gene that is mutated in familial cylindromatosis, an autosomal dominant predisposition to tumors of skin appendages. Reduced CYLD expression has been observed in other tumor entities, including hepatocellular carcinoma. Hepatocellular carcinoma: one world, one cancer—different guidelines?

Hepatocellular carcinoma (HCC) is a healthcare challenge in virtually all parts of the world. Therefore, it is no wonder that different guidelines have been developed reflecting expert consensus on the management of HCC in different regions, namely guidelines from the American Association for the Study of Liver Diseases (AASLD) for the USA (1,2), the Asian Pacific Association for the Study of the Liver (APASL) for the Asia-Pacific region (3), the European Association for the Study of the Liver (EASL) for Europe (4), and the Japan Society of Hepatology (JSH) for Japan (5,6). In 2018 an update of the EASL clinical practice guidelines (CPG) will appear.

The diagnosis of hepatocellular carcinoma (HCC) can be reached noninvasively on imaging in patients at high risk as discussed in chapter “Hepatocellular carcinoma: Diagnostic Guidelines.” In this chapter, we describe the CT and MR imaging criteria more commonly used in the guidelines for the diagnosis and management of patients with HCC, including arterial phase hyperenhancement, washout, capsule, growth, and hypointensity on hepatobiliary phase MR imaging. AIMS We assessed the morbidity, mortality and efficacy of cryotherapy for primary and secondary malignant liver tumours in a prospective case control study. METHODS Since 1996 we performed 77 cryosurgical procedures on 71 patients. 6 patients had hepatocellular carcinoma, the remainders metastases, mainly of colorectal origin (n = 49). Cryotherapy was used when a complete tumour resection was not feasible, but complete tumour destruction by cryotherapy seemed possible. Mean follow-up was 21 months.
