Morphomechanics of the humero-ulnar joint: I. Joint space width and contact areas as a function of load and flexion angle

BACKGROUND Previous studies have shown that the trochlear notch is deeper than necessary for an exact fit with the humerus. However, humero-ulnar joint space width and contact areas have so far not been quantified for variations in the load and angle of flexion. METHODS Six fresh cadaveric specimens were investigated at 30 degrees, 60 degrees, 90 degrees, and 120 degrees of flexion and at loads of 25 and 500 N, simulating resisted elbow extension. Optimal positioning of the humeral component in the reverse shoulder prosthesis.

Total reverse shoulder arthroplasty is becoming more and more the standard therapeutic practice for glenohumeral arthropathy with massive lesions of the rotator cuff. The biomechanical principle of this prosthesis is represented by the reversion of the normal anatomy of the shoulder joint. This non-anatomical prosthesis leads to a medialization of the rotation centre of the glenohumeral joint and also to a distalization of the humeral head. All that causes a deltoid tension increasing so allowing a larger abduction of the arm. MR-Tomographie versus CT-Arthrographie bei glenohumeralen Instabilitäten

In a prospective study the diagnostic value of MRI compared to CT arthrography (CT-A) was evaluated in 26 patients with 27 instable shoulder joints. Surgical and/or arthroscopic correlation was available in all cases. Both methods showed a high accuracy (96% CT-A, 94% MRI) in the evaluation of the glenoid labrum. CT-A was significantly superior to MRI in the detection of capsular lesions (sensitivity 96% vs. 44%, accuracy 96% vs. 72%, negative predictive value 96% vs. 67%). Aston Medical Duocentric® Reversed Prosthesis: Proposition of New Glenoid Component Design to Prevent Scapular Neck Notching

Medialization of the center of rotation in reverse shoulder arthroplasty (RSA) results in greater excursion of the proximal humerus especially in adduction leading to a mechanical conflict against the neck of the scapula, thus creating a specific complication: scapular notching, in up to 90 % of RSA. We describe our third generation of RSA in Dijon, the Duocentric prosthesis. This prosthesis provides a new proposition to prevent the notching problem with an inferior overhang integrated to the glenoid baseplate. Distal humerus fractures display seldom but severe injuries because of the complex distal humerus anatomy and often comminuted fracture types. They account for approximately 2–3 % of all fractures and for 17–30 % of fractures around the elbow. In younger patients, there is a predominance among males. The mechanism of accident is mostly a high-energy trauma in this population. In contrast, among elderly patients, distal humerus fractures concern mostly women with osteoporotic bone.
