Obstructive uropathy following aortofemoral graft surgery.

Fifty patients who had undergone aorto-bifemoral bypass with a bifurcated Dacron graft for aortoiliac arteriosclerotic obliteration were examined with real-time sectorial ultrasound to screen for the presence of hydronephrosis. Hydronephrosis was detected in only 1 case. The low incidence of this complication in our cases may be due to the fact that all our patients were explored at least 1 year after surgery. Some may have developed hydronephrosis early, but this may have resolved spontaneously during the first year after the operation, thus remaining undiagnosed. Lithiasic obstructive uropathy. Hydronephrosis characterization by magnetic resonance pyelography.

The purpose of this study was to evaluate the role of magnetic resonance (MR) pyelography in patients affected by hydronephrosis due to ureteric stones, in order to identify a pyonephrotic condition.In the last 3 years, 315 patients, who had originally been investigated by ultrasonography, were evaluated with MR pyelography in order to define the etiology of obstruction. Percutaneous nephrostomy in the treatment of hydronephrosis during pregnancy: Our experience

Hydronephrosis during pregnancy is commonly considered physiological due to compression of the ureters by the gravid uterus and by dilated ovarian veins. Only in symptomatic cases or secondary to obstruction is a urinary diversion required. Six cases of symptomatic hydronephrosis treated with percutaneous nephrostomy are reported. Tocolytic therapy was practised in order to avoid the risk of abortion.

End Cutaneous Ureterostomy for the Management of Severe Hydronephrosis

Purpose: Although rarely indicated, surgical treatment of severe megaureter can pose a formidable technical challenge, especially in the small infant. We present our experience and outcomes with end cutaneous ureterostomy as a temporizing adjunct to future ureteral reimplantation. Materials and Methods: We performed a retrospective cohort study of patients who underwent end cutaneous ureterostomy between 1993 and 2005. Real-time ultrasound in Crohn's disease: characteristic features and clinical implications.

Thirty-two children with Crohn's disease were evaluated by real-time ultrasonography. The typical pattern of Crohn's disease caused by bowel wall thickening is the “bull's eye phenomenon”, the elongated tubular stiff bowel loop with narrowing of the lumen and the small bowel conglomerate tumor. In indefinable abdominal complaints sonography may lead to the correct diagnosis. The differential diagnosis of similar sonographic features and the limitations of ultrasound in gastrointestinal disease must be considered. Unilateral ureteral obstruction (UUO) has been used as an experimental model to induce tubulointerstitial damage and interstitial fibrosis. UUO is characterized by cellular proliferation, accumulation of inflammatory cells, and subsequent replacement of renal parenchyma by fibrous tissue. The influx of inflammatory cells into the renal interstitium is mediated by adhesion molecules. Treatment with phenoxybenzamine of upper urinary tract complications caused by intravesical obstruction.

Five children in whom reflux and progressive hydronephrosis persisted despite multiple surgical attempts or repair are described. In all cases cystomanometry displayed a marked elevation of the bladder outlet resistance combined with high intravesical pressure values. Therapy with phenoxybenzamine, an alpha-adrenergic blocker, was successful in all cases, restoring a free urine passage of the upper urinary tract and unimpaired voiding preventing urinary diversion which has been considered in some of these children. Although there were no signs of bladder neuropathy, a hyperfunction of the sympathetic innervation as acause for bladder complications is discussed.

Antibiotic prophylaxis in antenatal nonrefluxing hydronephrosis, megaureter and ureterocele

Observation is a conservative management option in infants with nonrefluxing hydronephrosis, primary nonrefluxing megaureter and ureterocele diagnosed postnatally following antenatal detection of hydronephrosis. Antibiotic prophylaxis might be a sensible regimen under these circumstances to prevent UTI in this population who are potentially at increased risk. However, studies examining the efficacy of prophylactic antibiotics are sparse in this setting. For each condition, prophylactic policies seem extremely variable, and UTI rates vary widely with comparable rates reported between patients followed on and off antibiotics.
