The basic structural lesion of persistent neonatal hypoglycaemia with hyperinsulinism: deficiency of pancreatic D cells or hyperactivity of B cells?

Pancreatic tissue obtained at subtotal pancreatectomy from 15 infants with persistent hypoglycaemia with hyperinsulinism, and autopsy specimens from 23 age-matched normoglycaemic controls, were studied with morphometric methods after immunocytochemical staining of the four main islet cell types (A, B, D and pancreatic polypeptide cells). In three cases, a focal lesion was detected by gross examination. Macroscopic or microscopic examination did not distinguish the 12 other cases from controls. OBESITY: A NEW PATHOLOGY TO PAY ATTENTION TO IN YOUNG PEOPLE

Obesity in young people is a form of malnutrition that is found more and more in industrialized countries known both for its association with obesity in adult age and the chronic-degenerate pathologies correlated to it. In 1998 in the United States the prevalence of obesity in 4-12 year olds was 22% for the Hispanics and Afro-Americans and 12.3% for the non Hispanic Caucasians Also in children as in adults, the consequence of obesity is hyperinsulinemia, in direct proportion to the body weight. In this review we discuss the effects of hyperinsulinemia and obesity in young people, and evaluated the difference of cardiovascular complication in young and in adults.

Mboat7 down-regulation by hyper-insulinemia induces fat accumulation in hepatocytes.

Background: Naturally occurring variation in Membrane-bound O-acyltransferase domain-containing 7 (MBOAT7), encoding for an enzyme involved in phosphatidylinositol acyl-chain remodelling, has been associated with fatty liver and hepatic disorders. Here, we examined the relationship between hepatic Mboat7 down-regulation and fat accumulation. Methods: Hepatic MBOAT7 expression was surveyed in 119 obese individuals and in experimental models. MBOAT7 was acutely silenced by antisense oligonucleotides in C57Bl/6 mice, and by CRISPR/Cas9 in HepG2 hepatocytes. Is preoperative radiographic localization of islet cell tumors in patients with insulinoma necessary?

Preoperative localization tests [sonography, computed tomography, angiography, percutaneous transhepatic portal venous sampling for insulin radioimmunoassay (PTP)] have a sensitivity of 60% to 90% in cases of organic hyperinsulinism. In all publications, however, the sensitivity of intraoperative localization, 75% to 100%, is distinctly higher. With the exception of PTP, all tumors that can be identified by preoperative localization can also be detected using palpation or intraoperative sonography. Risposta insulo-secretrice al carico endovenoso di tolbutamide

Gli AA. hanno studiato il comportamento della risposta insulinemica dopo stimolazione con tolbutamide endovenosa in un piccolo gruppo di soggetti normali normosomici, obesi non diabetici e diabetici obesi. In tutti i soggetti il massimo aumento dell'insulinemia (determinata con metodo radio-immunologico) compare a 5 mp ed e maggiore negli obesi rispetto ai normali e ai diabetici. La caduta percentuale del picco insulinemico dimostra un andamento parallelo nei normali e negli obesi, mentre e molto piu lenta nei diabetici obesi. Vengono discusse le cause di questo comportamento e la natura dell'iperinsulinismo presente nell'obesita con e senza diminuita tolleranza agli idrati di carbonio.

Hyperinsulinemia, hyperproinsulinemia and insulin resistance in the metabolic syndrome.

For better comprehension of the metabolic syndrome, it is necessary to differentiate the effect of insulin on glucose metabolism on the one hand, and on other metabolic activities on the other hand. Whereas glucose utilization is affected by insulin resistance, the effect of insulin on lipid metabolism, ion and aminoacid transport does not seem to be diminished. Lipid metabolism, however, seems to play a crucial role in the induction of the vicious cycle. Increased energy and fat ingestion may be due to an increased number of galanin secreting cells in the hypothalamus.
