Introduction and objectives. The Charlson comorbidity index (CCI), an indicator of comorbidity, has been used as an adjusting variable in multivariate models. Because of its prognostic value per se for cardiovascular complications after acute myocardial infarction (AMI), we sought to determine the predictive value of the CCI for allcause mortality and recurrent AMI 30 days and 1 year after the index event. SIRT1 regulation of insulin-signalling pathways in liver, white adipose tissue and pancreas during fasting or calorie restriction

In an excellent review by Yang et al.[1], published in issue 5 of Trends in Endocrinology and Metabolism, the involvement of the human sirtuin SIRT1 in nutrient-sensing and insulin-signalling pathways is explained. The regulation of SIRT1 with fasting in liver, pancreas and white adipose tissue is illustrated (see Figure 2 of Yang et al.). We consider that the depiction in the article by Yang et al. could be misleading for the reader, and we propose a modified version (Figure 1).

Cocaine-related oronasal communication and hard palate destruction

Four cases of midpalatal perforation in cocaine abusers are presented. Other potential etiological processes are discussed to establish an adequate differential diagnosis. These patients were treated at our department, due to the drawback promoted by the establishment of oronasal communication that was provided after an accurate diagnosis. Histopathological evaluation of the lesion margins were conducted in two of the four cases, and yielded no evidence of vasculitis or active cocaine abuse. Therapeutic approach consisted of reconstructive surgeries and/or sealing prostheses.
