Isolated Endometriosis on the Rectus Abdominis Muscle in Women without a History of Abdominal Surgery: a Rare and Intriguing Finding

We report 2 rare cases of endometriosis on the rectus abdominal muscle diagnosed incidentally during an operation for inguinal hernia repair in women with no surgical history. Two women sought medical attention for a mass found in the pubic abdominal wall. Only 1 woman reported occasional pain. At physical examination in both women, an ovoid swelling in the right pubic area was felt. One woman experienced pain on palpation, and one reported slight discomfort. Ultrasonography demonstrated a heterogeneous hypoechogenic formation with indistinct edges; diagnosis was difficult. AIM: Effectiveness of surgery on quality of life in elderly affected by minimally symptomatic inguinal hernia. MATERIAL OF STUDY: Forty male patients aging over 75 years affected by minimally symptomatic inguinal hernia were included. In the first group were allocated 15 patients who refused hernioplasty; in the second were included 25 patients who underwent early elective hernioplasty using an high density polypropylene mesh under local anesthesia. Despite the high incidence of inguinal canal hernia disease in the population with approximately 20 milion patients that undergo hernia repair annually, Amyand's hernia is a rare clinical conditions. We report the first observed case of a young man who suffered of large and unsuspected left bowel ischemia following an elective right open hernioplasty. A 54-year-old man had a 2-year history of right inguinal reducible mass and was admitted to hospital for an elective day case open inguinal hernioplasty for a direct right inguinal hernia. Apart from mild hypertension controlled with ACE inhibitor, he was medically fit and well. The patient was submitted to open tension-free mesh repair with polypropylene preshaped mesh with local infiltration anesthesia and additive sedation with midazolam. Background Inguinal hernia represents one of the most common diseases in the elderly. It causes aching and unsightly swelling affecting quality of life (QOL), with a concomitant risk of strangulation. Despite this, an assumption of the risks of surgery may lead clinicians, particularly non-surgeons, to advise elderly patients against the repair of hernia, especially if asymptomatic [1]. The aim of this study was to evaluate QOL by a short Form 36 (SF-36) questionnaire [2] in elderly patients undergoing inguinal hernioplasty.
