Summary We have previously reported that the magnitude of deep inspiration (DI)-induced bronchodilation is only slightly reduced in mild asthmatics, compared to healthy subjects. The aim of this study was to evaluate whether increased severity of asthma is associated with impairment in the ability of DI to induce changes in lung volume. We previously reported that responsiveness to methacholine (Mch) in the absence of deep inspiration (DI) decreased in healthy subjects after a short course of exercise training. We assessed whether a similar beneficial effect of exercise on airway responsiveness could occur in asthmatics. Bronchial hyperreactivity seems to induce a loss of the deep inspiration's bronchoprotective effect which is normally able to dilate the obstructed airways. Studies such as the HRCT on the bronchial gauge's alterations show that the deep inspirations bronchodilator effect is associated with the bronchial distensibility. In fact, it is almost totally absent either in the elderly or in the COPD patients. In asthma, instead, the pathology's seriousness has proved to be associated with the impairment of the deep inspiration's broncho-protective effect which this way becomes an important diagnostic parameter.

Use of a portable device to record maximum inspiratory flow in relation to dyspnoea in patients with COPD

SummaryForced inspiratory measures have been described to reflect the reduction in dyspnoea upon bronchodilation in severe COPD. Based on this we evaluated the applicability and usefulness of a portable device for the assessment of forced inspiration. In 37 patients with COPD (GOLD II/II/IV n = 16/15/6, mean ± SD FEV1 46.2 ± 15.4%pred) lung function was recorded prior to inhalation of 24 μg formoterol and 30 min later. Assessments comprised spirometry including forced inspiration, body plethysmography, maximum inspiratory flow (InCheck, Clement Clarke), and changes in dyspnoea via visual analogue scale (VAS).
