Sociodemographic factors as determinants of quality of life and quality of social interactions among heterosexual couples undergoing assisted reprodu…

2021

Introduction When facing with stressful physical and psychosocial events, sociodemographic factors can be an important factor for quality of life and quality of social interactions [1]. The experience of infertile couples undergoing ART is a complex and stressful process that may have a negative effect on quality of life [2]. There is a need to better understand how certain sociodemographic factors impact on the quality of life and in the quality of social interactions of patients undergoing ART, in order to be able to anticipate the consequences from an integrated care perspective.MethodsThis study aims to assess the relationship between sociodemographic factors (sex, marital status and em…

Effectiveness of appropriate care pathways for frail older adults: a pilot study in Valencia (Spain)

2021

Introduction Frail older adults often have multiple health and social problems [1] but experience reduced care coordination due to fragmented service provision [2], and therefore are in need of integrated care. A comprehensive model integrating the provision of health and psychosocial care can promote health and wellbeing among frail older adults. This study explored the effects of a comprehensive model (APPCARE model) on health and wellbeing, and the healthcare resources use, of +75 frail older adults with complex and co-morbid clinical situations after their hospitalization. Methods Within the APPCARE model, two specific care pathways were designed and piloted in the city of Valencia (Spa…

Understanding frailty: meanings and beliefs about screening and prevention across key stakeholder groups in Europe

2017

ABSTRACTInnovative methods to manage frailty are critical to managing the needs of an ageing population. Evidence suggests there are opportunities to reverse or prevent frailty through early intervention. However, little is known about older adults’, families’ and practitioners’ beliefs about the malleability of frailty. This study examined European stakeholders’ accounts of the acceptability and feasibility of frailty screening and prevention to inform future intervention development. Semi-structured focus groups and individual interviews were conducted in three European Union countries (Italy, Poland and the United Kingdom) with key stakeholders – frail and non-frail older adults, family …

Activation of Stratification Strategies and Results of the interventions on frail patients of Healthcare Services (ASSEHS) DG Sanco Project No. 2013 …

2014

Abstract To face the challenge of active and healthy ageing (AHA), European Health Systems and services should move towards proactive, anticipatory and integrated care. Health care systems thus need to personalize services, put patients at the centre of care and provide services using the adequate resources. Population health risk management is emphasized through the use of tools to stratify people with chronic diseases according to their risk. Effective screening of frailty is vital for optimizing the care of frail populations at risk. The Activation of Stratification Strategies and Results of the interventions on frail patients of Healthcare Services (ASSEHS) EU project (N° 2013 12 04) is…

An integrated care pathway for cancer patients with diabetes: A proposal from the Italian experience

2020

Diabetes and cancer frequently coexist in the same subject, often with relevant clinical effects on the management and prognosis of the comorbid patient. The existing guidelines, however, do not appropriately address many clinical issues in this setting. Although collaboration between diabetologists and oncologists should play an important role in achieving appropriate levels of care, close coordination or agreement between these specialists is seldom offered. There is an urgent need for greater interdisciplinary integration between all specialists involved in this setting, for a shared approach ensuring that organisational silos are overcome. To this end, the Italian Associations of Medica…

Can Integrated Care Help in Meeting the Challenges Posed on Our Health Care Systems by COVID-19? Some Preliminary Lessons Learned from the European V…

2020

The COVID-19 pandemic puts health and care systems under pressure globally. This current paper highlights challenges arising in the care for older and vulnerable populations in this context and reflects upon possible perspectives for different systems making use of nested integrated care approaches adapted during the work of the EU-funded project VIGOUR ("Evidence based Guidance to Scale-up Integrated Care in Europe", funded by the European Union's Health Programme 2014-2020 under Grant Agreement Number 826640).

Promoting coordination in Norwegian health care

2011

This is the journal's version originally published in International Journal of Integrated Care. This article is designet as ”Open Access”. © International Journal of Integrated Care: http://www.ijic.org/index.php/ijic/index Introduction: The Norwegian health care system is well organized within its two main sectors - primary health and long term care on the one hand, and hospitals and specialist services on the other. However, the relation between them lacks mediating structures. Policy practice: Enhancing coordination between primary and secondary health care has been central in Norwegian health care policy the last decade. In 2003 a committee was appointed to identify coordination problem…

The B3 Maturity Model, SCIROCCO Tool and SCIROCCO Knowledge Management Hub: facilitating the capacity-building support for integrated care

2021

Introduction SCIROCCO Exchange is an EU Health Programme project that builds upon the preliminary achievements of the B3 Action Group on Integrated Care of the European Innovation Partnership on Active and Healthy Ageing (EIP on AHA) that first developed the concept of an Integrated Care Maturity Model. Through the activities of the SCIROCCO project, the Model has been further refined and is supported by a validated self-assessment tool (SCIROCCO Tool). SCIROCCO is an online participatory tool that helps stakeholders to understand: 1) The context requirements of a particular good practice; 2) The level of maturity required for a care system to adopt and scale-up integrated care; 3) The acti…

Enhancing the SCIROCCO tool using quantitative and qualitative methods

2018

Background: The SCIROCCO tool, developed as part of the SCIROCCO project, incorporates the Maturity Model for the adoption of Integrated Care developed by the European Innovation Partnership on Active and Healthy Ageing (B3 Action Group on Integrated Care). The online self-assessment tool can be used to facilitate scaling-up of integrated care across different health and care sectors, at different levels in health or care systems, and in a range of communities. Currently, more than 30 European regions are involved in testing the SCIROCCO tool. Since the project launch in April 2016, the tool underwent two iterations. Each of the iterations of the tool was followed by a round of refinement t…

Planning integrated care at primary care centres: lessons learnt through research in stratification tools in the Valencian Healthcare System

2018

Introduction and policy context: In the Valencian Region social services and the healthcare system are not connected. Also there is not any normative jointly regulating resources, services or aid benefits that would mean an integrated care IC portfolio.  Primary care PC centres are the closest and most trustworthy care facility for patients in the Spanish context. Also, PC is one of the most suitable resources to address needs of complex patients, as elderly and/or those with chronic conditions, through an IC approach, but there are some barriers that block its real and effective deployment. Objective and target population: Authors have studied the viability, implementation and development …
