EP1168 The influence of obesity on tumor recurrence in vulvar cancer patients

2019

Introduction/Background Obesity is associated with decrased survival in several cancer entities. The influence of obesity on prognosis of vulvar cancer patients is not clear. However, knowledge about this may have consequences on treatment and follow-up. Methodology This is an analysis of the large AGO-CaRE-1 study which included vulvar cancer patients (UICC stage IB and higher), treated in 29 cancer centers between 1998 and 2008, in order to analyze treatment patterns and prognostic factors. Results In total, 849 eligible patients were divided into two groups depending on their body mass index (BMI, 0.05). Radical vulvectomies were performed more frequently in patients having a BMI ≥30 kg/…

Effectiveness of a home exercise programme in low back pain: a randomized five-year follow-up study

2007

Background and Purpose. Therapeutic exercise has been shown to be beneficial in decreasing pain and in increasing functioning in patients with chronic low back pain. However, longitudinal follow-up studies are small in number, and often limited in the numbers of subjects due to drop-outs. In addition there is a shortage of real control groups in most cases. The purpose of the present study was to describe long-term changes in intensity of low back pain and in functioning for two study groups five years after undertaking a home exercise programme. Method. This was a randomized follow-up study over five years. Fifty-seven subjects were reassessed with questionnaires five years after their ini…

Combining Body Mass Index With Measures of Central Obesity in the Assessment of Mortality in Subjects With Coronary Disease

2013

Objectives This study sought to assess the mortality risk of patients with coronary artery disease (CAD) based on a combination of body mass index (BMI) and measures of central obesity. Background In CAD patients, mortality has been reported to vary inversely with BMI (“obesity paradox”). In contrast, central obesity is directly associated with mortality. Because of this bi-directional relationship, we hypothesized that CAD patients with normal BMI but with central obesity would have worse survival compared with subjects with other combinations of BMI and central adiposity. Methods We included 15,547 participants with CAD who took part in 5 studies from 3 continents. Multivariate stratified…

Secukinumab induction and maintenance therapy in moderate-to-severe plaque psoriasis: a randomized, double-blind, placebo-controlled, phase II regime…

2013

Abstract Background:  Interleukin-17A has major proinflammatory activity in psoriatic lesional skin. Objectives:  Assess the efficacy and safety of secukinumab, a fully human IgG1κ monoclonal anti-interleukin-17A antibody, in moderate-to-severe plaque psoriasis in a phase II regimen-finding study. Methods:  404 patients were randomised to subcutaneous placebo (67 patients) or one of three secukinumab 150 mg induction regimens: Single (week 0; 66 patients), Early (weeks 0, 1, 2, 4; 133 patients), and Monthly (weeks 0, 4, 8; 138 patients). The primary outcome was ≥75% improvement from baseline Psoriasis Area and Severity Index (PASI) score at week 12. PASI 75 responders from active treatment …

Renal resistive index: Beyond the hemodynamics

2020

We read with great interest the original article entitled “Prognostic significance of the renal resistive index in the primary prevention of type II diabetes” by Delsart et al1 In this issue of The Journal, the authors interestingly underlined the prognostic value of renal resistive index (RRI) in diabetic subjects and its role as an independent predictor of a first cardiovascular or renal event. Similar conclusions were previously obtained by others in different subsets of patients (hypertension, chronic kidney disease, or heart failure), in which intrarenal hemodynamic alterations showed to be independent predictors of cardiovascular events. It is likely that the mechanisms underlying ren…

A matter of fat: insulin resistance and oxidative stress

2012

Background Obesity is linked to insulin resistance (IR), which can lead to type 2 diabetes mellitus. Oxidative stress present in early obesity may favor the progression to comorbid conditions. Objective To examine the relationship between oxidative stress biomarkers and the severity of IR in a group of obese children. Methods Forty obese children with a body mass index (BMI) Z-score ≥ 2 were divided into two groups using the median obtained for the homeostasis model assessment of IR (HOMA-IR). Anthropometric parameters (including body fat composition by bioelectrical impedance) and biochemical parameters were assessed. The following biomarkers of oxidative stress were measured: malondialdeh…

Glycaemic variability using continuous glucose monitoring and endothelial function in the metabolic syndrome and in Type 2 diabetes

2010

Diabet. Med. 27, 872–878 (2010) Abstract Aims  Subjects who are at increased risk of developing diabetes may have increased glycaemic variability associated with endothelial dysfunction and possibly subclinical atherosclerosis, which may lead to increased cardiovascular risk observed at the time of diabetes diagnosis. To investigate this hypothesis, we measured endothelial function, carotid intima-media thickness and glycaemic variability using 48-h continuous subcutaneous glucose monitoring in 3 groups of overweight or obese subjects – those without the metabolic syndrome, and those with the metabolic syndrome with or without newly diagnosed Type 2 diabetes. Methods  Consecutive subjects, …
