A number of systemic disorders increase patient susceptibility to periodontal disease, which moreover evolves more rapidly and more aggressively. The underlying factors are mainly related to alterations in immune, endocrine and connective tissue status. According to the Italian Judicial System, a child under 14 years of age is not criminally responsible. A minor, less than 18 but over 14 years old, can bear criminal responsibility only if competent. This paper discusses some of the clinical aspects of establishing sanity/competency (‘compos mentis’) or insanity/ incompetency (‘non compos mentis’) and the legal and clinical implications of declaring an adolescent offender insane – Can he/she be imprisoned? Hospitalized? Should he/she be treated any differently than a non minor under the same circumstances?

We recently described a unique plasma metabolite profile in subjects with pediatric acute-onset neuropsychiatric syndrome (PANS), suggesting pathogenic models involving specific patterns of neurotransmission, neuroinflammation, and oxidative stress. Here, we extend the analysis to a group of patients with autism spectrum disorder (ASD), as a consensus has recently emerged around its immune-mediated pathophysiology with a widespread involvement of brain networks.
