Polycystic ovary syndrome (PCOS) is the most common endocrine disorder in females, with a high prevalence. The etiology of this heterogeneous condition remains obscure, and its phenotype expression varies. Two widely cited previous ESHRE/ASRMsponsored PCOS consensus workshops focused on diagnosis (published in 2004) and infertility management (published in 2008), respectively. Reproductive System Outcome Among Patients with Polycystic Ovarian Syndrome.

Polycystic ovarian syndrome (PCOS) may present with different clinical patterns and the anovulatory phenotype may not be the most common. Data suggest that anovulation in PCOS is not the consequence of increased androgen ovarian secretion but rather of a severe derangement of early follicle development. Other mechanisms may be operative in subgroups of patients and may contribute to the arrest of follicle growth and anovulation. At least 50% of anovulatory patients with PCOS become ovulatory in their late reproductive age. There is also evidence that menopause may occur later in women with PCOS. Finally, a strategy for treatment of infertility in PCOS is presented.

Obesity is associated with changes in the resolution of acute inflammation that contribute to the clinical complications. The exact mechanisms underlying unresolved inflammation in obesity are not fully understood. Adipocyte death leads to pro-inflammatory adipose tissue macrophages, stimulating additional adipocyte apoptosis. Thus, a complex and tightly regulated process to inhibit inflammation and maintain homeostasis after adipocyte apoptosis is needed to maintain health. Metabolic Consequences of Obstructive Sleep Apnea

The effects of OSA on metabolism are quite complex, and obesity, diabetes, nonalcoholic fatty liver disease and dyslipidemia often coexist with OSA, making it difficult to tease out the independent effects of OSA on metabolic variables. There is firm experimental evidence of a detrimental impact of OSA on adipose tissue dysfunction, with intermittent hypoxia, sympathetic hyperactivity, and oxidative stress as the most important intermediate mechanisms. Sleep fragmentation or deprivation may also contribute to the metabolic dysfunction of OSA. This work reviews the current knowledge on this topic, as well as the results of studies on the effects of CPAP treatment.

Single nucleotide polymorphisms near the Ink4/Arf locus have been associated with type-2 diabetes mellitus. Previous studies indicate a protective role of the locus in the carbohydrate metabolism derangement associated with ageing in wild-type mice. The present study demonstrates that the increased Ink4/Arf locus expression in 1-year-old mice, partially-deficient for the insulin receptor substrate (IRS)2 (Irs2 +/-SuperInk4/Arf mice) ameliorates hepatic steatosis, inflammation and insulin resistance.
