ADDISC lumbar disc prosthesis: Analytical and FEA testing of novel implants

The intact intervertebral disc is a six-freedom degree elastic deformation structure with shock absorption. 'Ball-and-socket' TDR do not reproduce these properties inducing zygapophyseal joint overload. Elastomeric TDRs reproduce better normal disc kinematics, but repeated core deformation causes its degeneration. We aimed to create a new TDR (ADDISC) reproducing healthy disc features. We designed TDR, analyzed (Finite Element Analysis), and measured every 500,000 cycles for 10 million cycles of the flexion-extension, lateral bending, and axial rotation cyclic compression bench-testing. The aim of this study was to check the balance between tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), and interleukin-10 (IL-10) in well-developed end-stage disk disease in the disk itself as well as in paradiskal spine. In 6 domestic pigs the cranial bony end plate of the L4 vertebra was perforated to the nucleus pulposus. At 3 months the degenerated experimental and contiguous control disks, together with the adjoining bony and cartilaginous vertebral end plates, bone marrow, and spinal ligaments, were excised and used for immunohistochemical analysis. Lumbar diskectomy is a widely performed surgical procedure in patients affected by lumbar disk herniation (LDH). Such a surgical procedure is considered to be safe and able to provide a consistent symptomatic relief in most patients. However, perioperative complications have been reported, ranging from 13% to 15%, and pose a considerable financial burden among patients, hospitals, and national health care agencies.
