Abstract Background Hepatic arterial embolisation therapy (HAET) is a treatment of liver metastases of gastrointestinal neuroendocrine tumours (GI-NETs). HAET increases circulating vascular endothelial growth factor levels. Everolimus is a treatment in NETs that may have antiangiogenic activity. Methods This phase II study was conducted in patients with predominant and progressive liver metastases from GI-NETs. Everolimus was initiated 7–30 days after HAET. The hypothesis was that everolimus after HAET would increase hepatic progression-free survival (hPFS) rate at 24 months from 35% to 50%. [18F]5-fluoro-2-deoxyuridine-PET for imaging of malignant tumors and for measuring tissue proliferation

The nucleoside 5-fluoro-2-deoxyuridine is a pyrimidine analogue accumulating in proliferative cells. We prospectively evaluated biodistribution of the PET tracer [(18)F]5-fluoro-2-deoxyuridine (FdUrd), its value for imaging malignant tumors, and its correlation to both [(18)F]2-fluoro-2-deoxyglucose (FDG)-PET findings and histological proliferation indices. Weekly Levofolinic Acid and 5-Fluorouracil Plus Hydroxyurea in Metastatic Gastrointestinal Adenocarcinomas

There were 42 patients with advanced gastrointestinal carcinomas (GA) enrolled in the study. In the Phase I part of the study we identified the MTD of 5-fluorouracil (5FU) in combination with levofolinic acid 100 mg/m2 per week intravenously plus hydroxyurea 1 g/m2 per week given by mouth in 3 refracted doses starting 6 hours after 5FU was administered. This treatment was given weekly for 6 consecutive weeks followed by a 15-day rest period. We were not able to increase 5FU weekly dosage above 700 mg/m2 due to the occurrence of grade 3-4 gastrointestinal toxicity. Evaluation of prognostic factors and their capacity to predict biological behavior in gastrointestinal stromal tumors.

Gastrointestinal stromal tumors (GISTs) are c-KIT-signaling-driven mesenchymal tumors of the human digestive tract, many of which have c-KIT or PDGFRα activating mutations. The authors studied the immunohistochemical markers, c-KIT and PDGFRα mutations, in GISTs and their association with the clinicopathological and clinical follow-up in 145 GISTs. Tumors were located mainly in the stomach, the median tumor size being 7.5 cm. The mitotic index was ≤5 mitoses per 50 high-power fields in 61% of cases, 96% expressed CD117, and c-KIT or PDGFRα mutations were detected in 68% of cases. Abdominal scintigraphy for diagnosis of intestinal bleeding

Abdominal scintigraphy with99mTc-pertechnetate, using a gamma-camera linked to a data processor, was done in 8 patients with acute massive intestinal hemorrhage and in 34 patients with chronic recurrent intestinal hemorrhage. Endoscopy and x-ray studies had failed in all patients to reveal a bleeding source. All but 3 patients underwent exploratory laparotomy, confirming the abnormal findings of scintigraphy. In all acutely bleeding patients, scintigraphy was positive. The bleeding was proven by laparotomy to be due to Meckel's diverticulum with gastric mucosa in 5, jejunal neurinoma in 1, and sigmoid colon carcinoma in 1 patient. Primary gastrointestinal involvement in non-Hodgkin's lymphomas.

This paper reviews primary gastrointestinal non-Hodgkin's lymphoma (GI-NHL). Every aspect of the topic is discussed though special attention is paid to histopathology and instrumental diagnosis as essential factors to stage the lymphoma and to determine an adequate therapy. Data from the most important works on the subject together with the results of our recent study of 40 primary GI-NHL are reported. Diverse findings by various authors are intentionally compared in a manner to present the work to the reader in the most critical way while trying to give an objective explanation of the different results on the basis of our own experience.
