The aim of this study was to compare the morphology of stable and unstable coronary lesions using intravascular ultrasound in patients undergoing coronary balloon angioplasty and to determine whether lesion morphology had any influence on the mechanism of balloon angioplasty. Thirty three (15 stable and 18 unstable) patients undergoing single lesion percutaneous transluminal coronary angioplasty were studied with intravascular ultrasound before and after intervention. Intravascular ultrasound is a new diagnostic modality which, for the first time, provides high resolution images of the vessel wall. 91 intraluminal ultrasound studies were performed in 50 patients during the course of 55 diagnostic and therapeutic catheterizations. A F-6 ultrasound catheter (20 MHz) was used. The method is an easily and rapidly performed addition to angiography. Changes in vessel walls (arteriosclerosis, dissection, tumour infiltration) can be readily diagnosed and their extent can be accurately described. The ability to determine luminal cross sectional area is particularly helpful for planning and performing therapeutic interventions.

Atherosclerosis is the leading cause of cardiovascular mortality and morbidity in the developed world. Intravascular ultrasound (IVUS) is a widely used imaging modality providing complementary diagnostic information to angiography regarding the vessel wall of the coronary arteries. IVUS has been used for assessment of ambiguous angiographic lesions, evaluation of new interventional devices and in atherosclerosis progression-regression trials. However, the standard gray-scale IVUS has limited value for the accurate identification of specific plaque components.
