Secondary Osseous Integration of Cementless Unicompartmental Knee Arthroplasty After Internal Fixation of a Felix Type IIIB Fracture

Case A 76-year-old man presented with periprosthetic tibial plateau fracture (TPF), with a completely loosened tibial component 3 weeks after cementless unicompartmental knee arthroplasty (UKA). Internal fixation by buttress plating was performed, and the tibial component was retained and left in situ primarily as a spacer. Revision was planned after fracture consolidation, but at 3 months, the patient was able to walk without support, without pain, and with full range of motion. At 1 year, he is free of complaints. Wear debris from ultra-high-molecular-weight polyethylene components used for joint replacement prostheses can cause significant clinical complications, and it is essential to be able to predict implant wear accurately in vitro to prevent unsafe implant designs continuing to clinical trials. The established method to predict wear is simulator testing, but the significant equipment costs, experimental time and equipment availability can be prohibitive. Unfortunately, many orthopedic surgeons believe that the definite solution for eliminating anterior knee pain in a young patient with multiple operations is the patellofemoral prosthesis (“The Great Solution”). But, they fail to ask themselves the most important question; what is causing the pain? When a patellofemoral prosthesis is implanted in these young patients without finding out the cause of the pain, a more serious problem could be created with a worse solution (“The Great Problem”). The cases we present spotlight this abuse.
