Initial viral load and decay kinetics of SARS-CoV-2 lineage B.1.1.7 in the upper respiratory tract of adults and children
2021
We read with interest the systematic review published by Walsh et al. in the Journal of Infection,1 focusing on the dynamics of SARS-CoV-2 RNA at the upper respiratory tract (URT). In this context, a novel SARS-CoV-2 variant lineage (B.1.1.7), first detected in the UK at the end of 2020 has transmission advantage over other lineages.2 Increased transmissibility of the B.1.1.7 variant has been linked to enhanced ACE2 affinity3 allegedly resulting in higher viral loads in URT, an observation that has been reported in some,3, 4, 5, 6 but not all7 large series published to date. In addition, longer duration of SARS-CoV-2 RNA shedding in URT has been reported in individuals infected by the B.1.1…
Molecular Epidemiology and Evolution in an Outbreak of Fulminant Hepatitis B Virus
2006
ABSTRACT In order to establish the transmission pathway for two outbreak patients affected by fulminant hepatitis B (FHB) following a shared period of hospitalization, we sequenced the complete genomes of the hepatitis B viruses (HBV) isolated from them as well as from the suspected common source and 11 additional controls. Phylogenetic and statistical analyses of these sequences revealed that the two FHB patients were indeed infected by a common source and that the fatal development of the disease did not appear to be associated with any mutation previously reported to be related to FHB. These data have also allowed us to estimate the extent and distribution of genetic variability along th…
A modified dinucleotide motif specifies tRNA recognition by TLR7
2014
RNA can function as a pathogen-associated molecular pattern (PAMP) whose recognition by the innate immune system alerts the body to an impending microbial infection. The recognition of tRNA as either self or nonself RNA by TLR7 depends on its modification patterns. In particular, it is known that the presence of a ribose methylated guanosine at position 18, which is overrepresented in self-RNA, antagonizes an immune response. Here, we report that recognition extends to the next downstream nucleotide and the effectively recognized molecular detail is actually a methylated dinucleotide. The most efficient nucleobases combination of this motif includes two purines, while pyrimidines diminish t…
Importance of sarcopenia parameter changes after living donor liver transplantation
2017
The systemic role of muscle tissue is strengthened by the large system of hormones, chemokines and other mediators that constitute a dense network of communication between the skeletal muscle and the liver (1,2). This, associated with the evidence of a progressive malnutrition and depletion of muscle mass in end-stage liver disease (ESLD) patients, has led many to study the role of sarcopenia and its systemic effects in this setting, and to identify it as critical risk factor for post- liver transplantation (LT) mortality (3-5). Englesbe and colleagues found a direct correlation between central sarcopenia, measured by computerized tomography (CT), the total area of the psoas muscle (psoas a…
Fumador, exfumador y COVID-19: la nicotina no protege contra el SARS-CoV-2
2021

2014
Sir,We have read with great interest the article in press ofAurilio et al., A meta-analysis of oestrogen receptor, pro-gesterone receptor and human epidermal growth factorreceptor 2 discordance between primary breast cancerand metastases (http://dx.doi.org/10.1016/j.ejca.2003.10.004), which will appear in Eur J Cancer. In the paper, the Authors have performed a meta-analysis of the stud-ies published in the literature concerning the discor-dance rate in oestrogen receptor (ER), progesteronereceptor (PgR) and HER2 status between primarytumour and corresponding relapse. It is well known thata considerable controversy concerns the issue of hor-mone receptors (HRs) expression as well as HER2 sta…

What is the prognostic importance of lymphovascular space invasion in the absence of lymph node metastasis for early-stage endometrial cancer?
2021
Objective: The aim of this study is to analyze the prognostic role of lymph-vascular space invasion (LVSI), evaluated in a semi-quantitative fashion on prognosis of early stage, low risk endometrial cancer (EC). Methods: We enrolled patients who underwent surgery for endometrial cancer between 2003 and 2018 in two referral cancer center. All patients had endometrioid EC, G1–G2, with myometrial invasion <50%, and no lymph-node involvement. LVSI was analyzed in a semi-quantitative way, according to a 3-tiered scoring system in absent, focal and substantial. Results: Among 524 patients, any positive LVSI was found in 57 patients (10.9%) with focal LVSI (n=35, 6.7%) and substantial LVSI (n=2…
