Low Back Pain: Guidelines for the Clinical Classification of Predominant Neuropathic, Nociceptive, or Central Sensitization Pain

Background: Low back pain (LBP) is a heterogeneous disorder including patients with dominant nociceptive (e.g., myofascial low back pain), neuropathic (e.g., lumbar radiculopathy), and central sensitization pain. In order to select an effective and preferably also efficient treatment in daily clinical practice, LBP patients should be classified clinically as either predominantly nociceptive, neuropathic, or central sensitization pain. Abstract Objective To determine the effectiveness of mechanical diagnosis and therapy (MDT) in patients with chronic low back pain (CLBP) compared to other traditional physical therapy interventions. Exercise has played a central role in rehabilitation of subjects with low back pain. The research in this field has intensified since the 1980s. Low back pain has been associated with both physically stressful and sedentary occupations. There is no clear association between low back pain and physical activity during leisure time. The current state of scientific knowledge of the biodynamic behaviour of the vertebral column under the influence of whole-body vibration whilst sitting and results from epidemiological studies caused the medical board of experts on occupational health of the Federal Ministry of Labour in Germany to add a new occupational disease to the official list of occupational diseases. Abstract Background and aim Low back pain (LBP) is a debilitating problem worldwide causing disability and reducing quality of life. The Fear Avoidance Beliefs Questionnaire (FABQ) was developed on the basis of the assumption that fear-avoidance beliefs play a major role in LBP-related disability.
