Purpose To assess the potential of 3-D spiral CT in the selection of adequate double-lumen tubes (DLT) for one-lung-ventilation. Materials and methods 3-D spiral CT of eight different DLTs was performed with standardized acquisition parameters: section thickness 4 mm, table speed 6 mm/s, increment 3 mm and standardised thresholds, window settings and magnification zoom. The accuracy was confirmed by measuring 3-D objects on screen and original DLTs by vernier caliper. 3-D spiral CT was performed in 20 patients preoperatively with slice thickness 4-8 mm, pitch 1-1.5, increment 4-8 mm. Klinische Erfahrungen mit dem Kensey-Katheter-System - Komplikationen und Ergebnisse

Fifteen procedures have been carried out in 14 patients using the Kensey rotation angioplasty system. Sixteen occlusions of the superficial femoral artery, with an average length of 10.2 cm (3 to 24 cm) and one artery with multiple stenoses were treated. The following results were obtained: creation of a lumen, with subsequent balloon dilatation in eight out of fifteen procedures; in two patients the vessels were occluded shortly after the procedure, due to complications (one obstructive plaque and one haematoma); five perforations, three dissections and one peripheral embolus. The total complication rate was 11 out of 15 cases, clinical improvement was obtained in six out of 15 cases.

A 48-year-old man developed progressively more severe epigastric pain, pain on pressure in the right upper abdomen and fever up to 38.6 degrees C so that acute cholecystitis was suspected. Ultrasound did not demonstrate a gall-bladder but a sickle shaped, dense echo with a distal adjoining echo-free zone. Computed tomography revealed air in the gall-bladder lumen as well as intramural and pericholecystic air pockets, findings pathognomonic for emphysematous cholecystitis. The aim of this study was to compare the morphology of stable and unstable coronary lesions using intravascular ultrasound in patients undergoing coronary balloon angioplasty and to determine whether lesion morphology had any influence on the mechanism of balloon angioplasty. AIM Should the sizes of balloons and stents used in the pelvic circulation be determined by the size of the ipsilateral or contralateral vessel? STUDY DESIGN The angiographic findings in 31 patients were analysed; in 10 an occlusion and in 21 a high grade stenosis (greater than 75%) of the common iliac artery had been treated by stenting (26) or simple PTA (5). RESULTS In 26 of the 31 patients there was marked reduction in the lumen distal to the lesion up to 40%. The ultra-thin bronchoscope in management of the difficult paediatric airway

The use of an ultra-thin flexible fiberoptic bronchoscope with a single lumen diameter of 2.7 mm at the distal tip to assist intubation of paediatric patients with a difficult airway is reported. Two patients (ages 30 months and 18 months) with mandibular hypoplasia and one patient (three months) with the Pierre-Robin syndrome are reported. In each case two fiberoptic bronchoscopes were used. The first allowed introduction of topical local anaesthetic while the second and smaller one was used for tube placement.

Objectives We aimed to evaluate the efficacy and safety of antegrade fenestration and reentry (AFR) for chronic total occlusion (CTO) recanalization in a multicenter registry. Background Adoption of antegrade dissection/reentry (ADR) for CTO recanalization has been limited, and novel ADR techniques are needed. Methods AFR involves the balloon-induced creation of multiple fenestrations between the false and true lumen. A targeted true lumen reentry is subsequently achieved with a low tip-load polymer-jacketed guidewire. Tietojärjestelmien suunnittelumenetelmiä kehitetään ja sovitetaan usein intuitiivisesti ja improvisoiden. Vaarana on, että näin syntyy vaikeasti ymmärrettäviä, tehottomasti sovitettavia ja huonosti soveltuvia suunnittelumenetelmiä. Improvisoivalle menetelmäkehitykselle on usein ominaista myös suurempi resurssitarve. Syynä tälle tilanteelle on se, että menetelmäkehitykseltä puuttuu kunnollinen menetelmätuki.
