Idarucizumab in High-risk Thoracic Surgery

Direct oral anticoagulants have suggested a favorable profile compared with vitamin K antagonists. However, the lack of treatment to reverse the effect of direct oral anticoagulants has limited its use in some patients who require rapid reversal of anticoagulation, as those included in the transplant waiting list. Idarucizumab is a recently approved drug to reverse the anticoagulant effect of dabigatran. However, the clinical experience when using this drug is scarce. Herein, we present a clinical case on anticoagulation reversal with idarucizumab to perform heart and lung transplantation in a patient with Eisenmenger syndrome.

A 65-year-old man was referred for evaluation of acute onset of fever, productive cough and dyspnoea. He had previously received a diagnosis of laryngeal carcinoma, which had been treated with laryngectomy and bilateral laterocervical lymphadenectomy, followed by chemotherapy. He underwent plastic surgery of the laryngocutaneous fistula, and a positron emission tomography (PET)- computed tomography (CT) examination performed during follow-up showed 18-FDG (2-fluoro-2-deoxyd- glucose) lung uptake in the apical right portion. This article examines the organisation of VOX, a new radical right party in Spain. It shows that the party has taken early and uneven steps to build a mass organisation and initially opted for open membership recruitment with participatory organisational elements. Also, the party’s rapid growth and quick entrance into political institutions at different state levels led the party leadership to establish more centralised control and limit members’ prerogatives, though recruitment continued. Una tecnica per migliorare le propriet`a meccaniche di tali giunti consiste nell’inserimento di rinforzi trasversali in forma di barre FRP. La presente memoria indaga il comportamento di travi lamellari sia da un punto di vista sperimentale che numerico, attraverso la formulazione di un semplice modello di interfaccia bifase (giunto adesivo e rinforzo).

Abstract The use of automatic systems in the analysis of medical images has proven to be very useful to radiologists, especially in the framework of screening programs, in which radiologists make their first diagnosis on the basis of images only, most of those corresponding to healthy patients, and have to distinguish pathological findings from non-pathological ones at an early stage. In particular, we are developing preprocessing methods to be applied for pulmonary nodule Computer Aided Detection in low-dose lung Multi Slice CT (computed tomography) images.
