The value of serial MR imaging in the assessment of brain metastases volume control during stereotactic radiosurgery

Purpose: To evaluate early tumour control capabilities of Stereotactic Radiosurgery (SRS) in the treatment of brain metastasis and the role of follow-up MR imaging. Methods and Materials: MR imaging of 54 metastases in 31 patients treated with SRS who underwent follow-up MR imaging within 12 months were retrospectively reviewed. Tumours were characterized as either enlarged (>20% volume increase), stable (follow-up volume ±20% of the initial volume), or decreased (> 0% volume decrease). Breast cancer is the most diagnosed cancer and the leading cause of cancer death in women. The 5-year relative survival rate is less than 30% for metastatic breast cancer and metastases account for more than 75% of breast cancer deaths. Fatty acid metabolism is altered in cancer and contributes to tumor progression and metastasis. The X-ray structure analysis reveals that 1 crystallizes in the monoclinic system with space group C2/c. The octahedral coordination around the metal center is made up of four distinct imine nitrogen atoms in the equatorial plane, and two sulfate oxygen atoms occupying the axial sites. A 42-year-old woman with right breast cancer cT4dN2M0 HR +, HER2 +, undergoes mastectomy with axillary lymphadenectomy after neoadjuvant chemotherapy. During adjuvant treatment with trastuzumab and hormone therapy, the patient develops multiple symptomatic encephalic metastases. She undergoes panencephalic radiotherapy and begins chemotherapy with capecitabine and lapatinib. After three months of well tolerated therapy, an important volumetric response of encephalic metastases is observed, with regression of neurological symptoms.

Bone metastases, often a consequence of breast, prostate, and lung carcinomas, are characterized by an increased bone turnover, which can be visualized by positron emission tomography (PET), as well as single-photon emission computed tomography (SPECT). Bisphosphonate complexes of 99mTc are predominantly used as SPECT tracers. In contrast to SPECT, PET offers a higher spatial resolution and, owing to the 68Ge/68Ga generator, an analog to the established 99mTc generator exists.
