Lingual nerve injury after third molar removal: Unilateral atrophy of fungiform papillae
2014
Background: Pain and sensory changes due to lingual nerve injury are one of the most common alterations that follow surgical removal of third molar. They are usually transient but other less common complications, such as the atrophy of fungiform papillae, have an uncertain prognosis. Case Description: We report a case of a 34-year-old woman who presented a unilateral lingual atrophy of fungiform papillae after third molar extraction accompanied by severe dysesthesia that altered her daily life significantly during the following months and how this complication evolved over time. We conducted a literature review on the different factors that can lead to a lingual nerve injury. Clinical Impli…
Association of Vital Pulp Therapy Outcomes with Tooth Type, Arch Location, Treatment Type, and Number of Surfaces Destroyed in Deciduous Teeth: A Ret…
2021
There is a paucity of information concerning vital pulp treatment outcomes in the undergraduate teaching setting. This study aimed to determine which type of deciduous molar, arch location, type of vital pulp therapy, and the number of carious surfaces involved had a better prognosis when carried out by undergraduate dental students. The method used was the review of clinical records of 590 patients with 600 deciduous molars, that visited the outpatient undergraduate dental clinics for vital pulp therapy. Statistical analysis used to determine the associations of tooth type, arch location, treatment type, and the number of carious surfaces involved in successful outcomes was logistic regres…
The effect of triangular and reversed triangular flap designs to post third molar odontectomy complications (a pilot study)
2020
Background Odontectomy is one of the most common surgical procedures in oral and maxillofacial surgery. Flap design influences the post operative complications. Triangular flap is the most widely used flap design but it has many shortages such as dehiscence, alveolar osteitis, reactionary bleeding, and periodontal disruption distal of second molar. The aim of this study is to introduce an alternative flap design in the surgical removal of impacted mandibular third molars – reversed triangular flap – and to compare this flap design with the triangular flap in case of dehiscence, reactionary bleeding, and clinical attachment loss. Material and Methods This prospective, split-mouth study invol…
The efficacy of 1% Betadine mouthwash on the incidence of dry socket after mandibular third molar surgery
2017
Background Dry socket or alveolar osteitis is a delayed healing of alveolar bone after exodontia causing moderate to severe pain 2-4 days after extraction of teeth. Antibacterial agents such as antibiotics and chlorhexidine have been previously proved to prevent or reduce the incidence of dry socket. Betadine is a mixture of iodine and povidone which has bactericidal, antifungal and antiviral effects. The aim of the present study was to evaluate the effect of preoperative povidone iodine 1% mouthwash before surgical extraction of impacted mandibular third molar, however age, gender and oral hygiene were also considered. Material and methods 189 patients who needed surgical extraction of Pel…
Radiographic signs associated with inferior alveolar nerve damage following lower third molar extraction
2009
The aim was to carry out a literature review of preoperative radiographic signs in orthopantomography (OPG) and computed tomography (CT) related with the risk of inferior alveolar nerve damage during the surgical extraction of lower third molar (LTM). A search was made on PubMed for literature published between the years 2000 and 2009. In the reviewed literature, radiographic signs in the OPG that indicate a relationship between the LTM and the inferior alveolar canal are considered a risk factor for nerve damage. These signs are darkening and deflection of the root, and diversion and interruption in the white line of the canal. In the majority of these studies, the routine use of CT is not…
Skillful management of exodontia complication to prevent dreadful sequelae
2011
Successful surgical treatments depend upon accurate diagnosis, judicious treatment planning, selection and execution of an appropriate surgical technique, and on a well monitored post-operative period. However, despite these precautions, extraction of teeth (particularly third molar) is exposed to the accidents and complications common to all buccomaxillary and facial interventions. A maxillary third molar may be displaced in maxillary sinus, nearby soft tissue, or may migrate in a superioposterior direction towards paraphyrangeal space, or infratemporal fossa. Dislocating the maxillary third molar in infratemporal fossa is one of the worst nightmare as this area has complex anatomy consist…
Obstetrics outcomes after complete and partial molar pregnancy: Review of the literature and meta-analysis.
2021
The hydatidiform mole is a rare gynecological disease rising from the trophoblastic. Post-molar pregnancies have an extremely variable course, varying from repeated abortions, stillbirths, preterm births, live births, or recurring in further molar pregnancies. Literature on obstetric outcomes following molar pregnancy is poor, often including monocentric studies, and with data collected from national databases. This review and meta-analysis aim to analyze the obstetric outcomes after conservative management of complete (CHM) and partial (PHM) molar pregnancies. The meta-analysis was performed following the Meta-analysis Of Observational Studies in Epidemiology (MOOSE) and the preferred repo…
Pericoronitis: A clinical and epidemiological study in greek military recruits
2019
Background This paper presents a statistical analysis of epidemiological, clinical and radiographical characteristics of third molar-related pericoronitis. Material and methods 650 conscripts of the First Training Division of Conscript Soldiers of 2005 in Greece were recruited for the study. Each conscript was given a questionnaire and underwent a clinical test and a radiographic examination. The tested variables included the conscripts' personal information, oral hygiene parameters along with the radiographic angulation of the third molar, the level of impaction and their classification in relation to the edge of the mandible. Results The prevalence of pericoronitis was found to be 4.92%. …
Extraoral surgical removal of an ectopic impacted third molar of the mandible. Report of a case
2020
Intraoral approach for the removal of impacted third molars represents a common surgical procedure for the specialized clinician. However, in some cases such as ectopic third molars, extraoral surgical removal seems to be inevitable. We present a step by step case of a 56 year old woman with an ectopic third molar of the lower jaw along with a cystic lesion, which were surgically removed by a submandibular approach. Postoperative clinical course was uneventful and there were no signs of facial nerve paresis. In such cases, appropriate preoperative planning must be made based on careful study of radiographic imaging and clinical examination. The more conservative technique that would minimiz…
Repercussions of intraalveolar placement of combination of 0.2% chlorhexidine & 10 Mg metronidazole gel on the occurrence of dry sockets- A randomize…
2017
BACKGROUND To evaluate the effects of intraalveolar placement of gel containing 0.2% chlorhexidine and 10gm of metronidazole on the incidence of alveolar osteitis. MATERIAL AND METHODS A total of 300 impacted third molars were extracted in 150 patients enrolled in this trial. In each subject a socket was randomly selected and packed to the crest of alveolar ridge with the gel. The contralateral socket was packed with placebo dressing. The occurrence of dry socket was assessed during 3rd and 5th postoperative days .The data was analysed using a meta analytical program. STUDY DESIGN Double blind, prospective, placebo controlled trial. RESULTS The combination of metronidazole + chlorhexidine g…
